2 SPECIFICATIONS FOR BUS OPERATOR UNIFORMS BUS OPERATOR UNIFORMS GENERAL: (1) In accordance with the Memorandum of Agreement with TWU Local 208, the Central Ohio Transit Authority will issue each operator in service (currently a total of 686), and each new operator, as of their going on board as an operator, on a one-time basis, a basic uniform consisting of the following items: Seven (7) shirts (combination of short or long sleeve and polo shirt) Seven (7) pants (or combination of approved skirts or culottes) One (1) winter coat One (1) all season coat One (1) sweater One (1) pair of uniform shoes One (1) hat One (1) Uniform belt Two (2) Uniform ties (2) Each operator who has been in continuous service of the Authority, as a regular operator, shall receive a voucher in the amount of five hundred ($500) to be used for purchasing uniform items at an authorized vendor contracted by the Authority or the option of obtaining a uniform allowance of $250 (COTA labor contract year 2018), to be paid in two equal installments of $125, instead of the $500 uniform voucher. New operators shall receive a voucher in the amount of $1,000 to be used for purchasing uniform items with the assistance from the Director of Transportation or their designated representative. (3) The voucher in the amount of $500 will be available each contract year. The voucher must be used in the contract year it was issued and unused balances cannot be carried forward, transferred to any other operator, used to purchase items for another person, or redeemed for cash. (4) COTA s anticipated operators will number 689 by the end of 2018, and this figure is expected to stay flat through September of A. UNIFORM CLOTHING MATERIAL: Uniform clothing shall be composed of the specifications listed in Paragraph D, pages 4 through 10. Contractor will be required to furnish all clothing as specified by COTA uniform requirements. B. MEASUREMENTS: Contractor shall be required to take all measurements to properly fit each employee for their uniforms prior to the issuance of clothing. All clothing shall be of a regular cut and loose fit for fitting purposes. III-1 (revised )

3 Both short or long sleeve and polo shirts provided shall be for year round use (employee s option). The contractor shall provide COTA Logo Patches to be sewn on the right sleeve of the shirt. The COTA approved embroidery or COTA appliqué for hats will be provided by the contractor upon request. The COTA appliqué will be a red 1 x ¾ oval with white wording to read COTA. Successful contractor must provide a sample for COTA s approval. Operator s option: Service stripes (provided by contractor) are worn centered on the outside bottom half of the right sleeve on the black uniform coat or long sleeve uniform shirt. The service stripe is placed at an angle of 45 degrees with the lower end toward the inside seam of the sleeve, and it is placed four inches from the bottom of the sleeve. For each additional period of three years service, another service stripe is added above and parallel to the first stripe, with a 1/16-inch space between stripes. The cost, per employee name, of applying of service stripes to COTA uniforms, when requested by COTA employee, shall be included in the price for the initial garment. Cost of subsequent additions of stripes will be borne by the employee. The contractor shall be required to provide alterations and affixing patches on an as needed basis. COTA will allow the cropping of women s shirts, for the initial order, for all women who elect to have these modifications made. This includes any new hires throughout the contract. The cost of these services may be applied to the voucher. C. ORDERING PROCEDURE: The contractor will be responsible for measuring and ordering, for each operator currently in service, and each new operator, as of their going on board as an operator, the specified uniform. When a new operator is hired and completes his/her eligibility for issuance of the required new uniform items, the Director of Transportation or his designated representative will prepare an order form (supplied by the vendor) detailing the items to be ordered for that operator using the voucher assigned to the new operator. The first order only for a new operator must be sent directly to COTA. Any subsequent orders for the operators are to be sent to the individual operator s home address. Each operator who has been in continuous service of the Authority, as a regular operator, shall receive a voucher in the amount of $500 or a uniform cash allowance of $ (COTA labor contract year 2018), to be paid in two equal installments of $125 provided by COTA to be used for purchasing replacement uniform items at the authorized vendor contracted by the Authority. This voucher is to be used for replacement of uniform parts as specifically mentioned in Section D, pages 4 through 10. Under the uniform voucher option, operators will be using the voucher to purchase all of their needed items under this contract. The voucher will be valued at the amount of each operator s clothing allowance for each year. The awarded contractor must manage the balances remaining for each operator for this payment option. If charges are made by a COTA operator that exceeds the amount of the voucher, the operator is responsible for the balance, not COTA. All transaction amounts over the spending limit for an operator will be handled strictly between the COTA operator and the sales representative involved, regardless if the purchase is made online or at contractor s physical location. 4 contractor agrees not to charge merchandise until the product ordered is actually delivered to the COTA operator. By submitting a bid, the contractor certifies they can provide a consolidated monthly invoice. PURCHASING PROCEDURES: If an individual should spend more than the allotted voucher amount, the contractor will be responsible to collect the difference from the operator at the time of the sale. Operators must show their COTA identification at the time of each purchase. Operators must have a voucher to purchase under this contract. COTA will not be liable for purchases made by Operator no longer employed by COTA and have turned in their identification prior to a purchase. Operators will be responsible for placing their order via the contractor s online store or at the contractor s physical location. The operator voucher will be used for payment but will not be charged until the items have shipped to the home address of the operator. COTA is responsible for reviewing the order for accuracy and contacting the contractor for any disputes/returns/exchanges. The successful contractor under this RFB will be responsible for providing and maintaining a computerized system that (1) tracks the individual operator s remaining allowance available for the current calendar year and (2) limits the operator to purchase only those items specifically listed and authorized as listed in Section D pages III-4 through III-10. Operators that select the cash allowance option will be responsible for ordering and paying for their uniform replacement items. A sample copy of the computer report is to be provided with the bid. Upon reaching his/her allowance for the calendar year, payment for any orders placed by an operator will be the personal responsibility of the operator. Additionally, any balance remaining on the voucher at the end of a calendar year, cannot be carried forward to pay for purchases in the following year. Operators cannot transfer any unused portion of their allowance to another operator. COTA uses the consolidated monthly invoice option, one (1) invoice will be issued monthly with a grand total cost of all purchases made within the invoice period. The invoice must include an attachment with the detail of all purchases and the cost per item. (Example: Ten (10) uniform shirts at the contracted cost with the extended cost, etc.). This must be sent electronically to accountspayable@cota.com. Payment terms will be NET 30 days. D. BUS OPERATOR UNIFORM SPECIFICATIONS Note: Contractor to add COTA logo patch to right sleeve 1 down from shoulder seam on all shirts/blouses, blazers, sweaters and jackets. (Patches to be provided by contractor.) Contractor to add service stripes 4 inches from the bottom of the right sleeve of uniform coat, long sleeve sweater or long sleeve shirt. DELIVERY OF UNIFORM ITEMS: (1) The Contractor shall package all items ordered for each individual and label the package with the employees name and contents. (2) All uniforms are to be delivered to the home address provided by the operator at the time of purchase. The only exception is for the first time order for new bus operators which will be sent to 1600 McKinley Ave., Columbus, Ohio E. STAFFING: Contractor shall provide a local account representative who will be authorized to react to problems arising during the contract term (e.g., delivery problems, returns, exchange etc.). The name of the local account representative must be submitted with the bid. The account representative will work directly with the operator to resolve all issues. Any issues with the new operator first time orders, the account representative will work with the Director of Transportation or his designated representative. G. COMPENSATION: Consolidated invoicing will be the method of compensation to the awarded contractor. H. RETURNS: Any uniform items which need to be returned/exchanged will be coordinated by COTA and the local account representative assigned to COTA. III-10 (revised )

12 I. ALTERATIONS: The Contractor shall provide COTA approved alterations and the affixing of all patches on all orders at no additional charge to COTA. The Contractor shall be required to provide a representative to take uniform measurements. Any subsequent alterations beyond those associated with the initial fit will be at the employees expense. If there are remaining voucher funds on the voucher, the employee may elect to use those to cover this expense. J. RECORDKEEPING: The Contractor agrees to maintain a record system for documentation and results of all services provided pursuant to this Request for Bids. The Contractor will provide documentation monthly and COTA will be able to access this information via the internet. K. CONTRACTOR LOCATION AND HOURS OF OPERATION: Contractor will have a retail facility located in Franklin County and will maintain normal business hours Monday through Friday with the exception of national holidays. Contractor must have merchandise on hand for employee fittings and measurements. L. MANUFACTURER LABELS: All clothing is to be supplied with the original manufacturer labels. Private/house labels are not acceptable. * Contractor will be provided with a list of those operators that are authorized to purchase the Tactical Shorts. III-11 (revised )

New supervisor initial uniform set up cost is Two Thousand and 00/100 Dollars ($2,000). Each Supervisor who has been in continuous service of the authority in need of replacement clothing shall be authorized for purchasing uniform items at an authorized vendor contracted by the Authority. Contractor shall provide internet capabilities to place orders and to track orders. DELIVERY OF UNIFORM ITEMS: 1. The Contractor shall package all items ordered for each individual and label the package with the employees name and contents. 2. All uniforms are to be delivered to the home address provided by the operator at the time of purchase. D. STAFFING: Contractor shall provide a local account representative who will be authorized to react to problems arising during the contract term (e.g., delivery problems, returns, exchange etc.). The name of the local account representative must be submitted with the bid. The account representative will work directly with the operator to resolve all issues. Any issues with the new operator first time orders, the account representative will work with the Director of Transportation or his designated representative. E. COMPENSATION: Consolidated invoicing will be the method of compensation to the awarded contractor. III-18 (revised )

20 F. RETURNS: Any uniform items which need to be returned/exchanged will be coordinated by COTA and the local account representative assigned to COTA. G. ALTERATIONS: The Contractor shall provide COTA approved alterations and the affixing of all patches on all orders at no additional charge to COTA. The Contractor shall be required to provide a representative to take uniform measurements. H. MANUFACTURERS LABELS: All clothing is to be supplied with the original manufacturer labels. Private/house labels are not acceptable. I. RECORDKEEPING: The Contractor agrees to maintain a record system for documentation and results of all services provided pursuant to this Request for Bids. The Contractor will provide documentation monthly and COTA will be able to access this information via the internet. I. CONTRACTOR LOCATION AND HOURS OF OPERATION: Contractor will have a retail facility located in Franklin County and will maintain normal business hours Monday through Friday with the exception of holidays.
