4 Product Application Guide Section 1 Prevention/Protection SureSkin II THIN INDICATIONS: Early Intervention at first sign of skin breakdown Dry superficial chronic/acute wounds Post wound healing protection ASSESSMENT: Determine nature of redness or superficial skin breakdown - Measure size of area involved (i.e. LxWxD) DRESSING SELECTION: Determine dressing size needed. Allow for at least a 1 to 1 1/4 inch (3 cm) border around the visible wound area involved and select the appropriate size SureSkin II THIN See Dressing Selection Guidelines page 12 DRESSING APPLICATION: 1 Clean wound area with normal saline or water 2 Pat area dry ( do not use ointments or moisturizers on skin) 3 With one hand, grasp paper release liner at end with arrows 4 Use other hand to grasp plastic film release liner at arrows 5 Turn dressing over, center dressing over wound area and press in place 6 Remove paper release liner completely and smooth in place 7 Remove plastic film release liner completely and smooth in place 8 Place hand over dressing and mold dressing to skin contours WHEN TO CHANGE DRESSING: A bubble often appears as the dressing interacts with wound fluid. Change the dressing before the bubble reaches the edge of the dressing. DRESSING REMOVAL: 1 With one hand gently lift edge of dressing from skin surface 2 Use other hand to press down on skin to separate dressing from skin 3 Use this technique to work your way around the dressing and loosen it completely from the skin 4 Gently roll dressing off skin 5 Alternatively, stretching the dressing horizontally and lifting it will also facilitate removal in most situations 6 Discard dressing and any packaging materials into appropriate receptacle NOTATION SureSkin II dressings are intended to remain in place up to 7 days depending on wound location, amount of exudate and skin condition. If dressing is intact and not leaking, it is not recommended to change the dressing a short time after application. 2

6 Section 2 4 NOTATION Partial/Full Thickness Wounds SureSkin II BORDER STANDARD INDICATIONS: Partial /full thickness chronic/acute wounds, I.e. Stage 2-4 pressure, venous or diabetic foot ulcer, Donor sites, 2 degree Burns, Mild to Moderate Wound drainage ASSESSMENT: Determine nature/stage of wound and exudate level Measure dimensions of area involved (i.e. LxWxD) DRESSING SELECTION: Determine dressing size needed. Allow for at least 1 to 1 1/4 in (3 cm) border around the visible wound area involved and select the appropriate size SureSkin II BORDER STANDARD dressing. Allow for at least 1 to 1 1/4 in (3 cm) border around the visible wound area involved and select the appropriate size SureSkin II dressing See Dressing Selection Guidelines page 12 DRESSING APPLICATION FOR SACRAL: 1) Clean wound area with normal saline or water 2) Pat area dry ( do not use ointments or moisturizers on skin) 3) With one hand, grasp paper release liner at end with arrows 4) Use other hand to grasp plastic film release liner at arrows 5) Turn dressing over, fold dressing in half and center the dress ing between buttock folds with narrow end of dressing pointing down 6) Remove paper release liner completely and smooth in place 7) Remove plastic film release liner completely and smooth in place 8) Place hand over dressing and mold into place WHEN TO CHANGE DRESSING: A bubble often appears as the dressing interacts with wound fluid. Change the dressing before the bubble reaches the edge of the dressing. 10 Section 3 Challenging Anatomical Locations SureSkin II ELBOW HEEL EAR DIGIT DRESSING APPLICATION FOR ELBOW HEEL: 1) Clean wound area with normal saline or water 2) Pat area dry ( do not use ointments or moisturizers on skin) 3) With one hand,grasp paper release liner at end with arrows dressing 4) Use other hand to grasp plastic film release liner at arrows 5) Turn dressing over, center dressing over wound on elbow and press in place 6) Smooth middle portion of dressing to skin 7) Remove paper release liner completely from dressing and fold over to form one side of a cup and smooth in place 8) Remove plastic film liner completely from dressing and fold over to form other side of cup and smooth in place 9) Place hand over dressing and mold dressing to skin contours DRESSING APPLICATION FOR EAR: 1) Clean wound area with normal saline or water 2) Pat area dry ( do not use ointments or moisturizers on skin) 3) Select smallest appropriate size dressing and cut as indicated 4) Remove release liners 5) Turn dressing over, center dressing over wound area and press in place on ear 6) Use fingers to mold dressing to ear contours DRESSING APPLICATION FOR DIGITS (TOE FINGER): 1) Clean wound area with normal saline or water 2) Pat area dry ( do not use ointments or moisturizers on skin) 3) Select smallest appropriate size dressing and cut as indicated 4) Remove release liners 5) Turn dressing over, center dressing over wound and press in place on toe 6) Use fingers to mold dressing to digit contour WHEN TO CHANGE DRESSING: A bubble often appears as the dressing interacts with wound fluid. Change the dressing before the bubble reaches the edge of the dressing. DR D 8 NOTATION DRESSING CHANGE AND REMOVAL FOR EAR DIGITS (TOE FINGER): Same as for sacral dressing SureSkin II dressings are intended to remain in place for up to 7 days depending on wound location, amount of exudate and skin condition. If dressing is intact and not leaking, it is not recommended to change the dressing a short time after application. Sometimes the edge of the dressings border may lift slightly. Securing the edge with tape may eliminate the need for full dressing change. In most cases, this is not needed.

12 Section 4 Venous Ulcers SureSkin II BORDER STANDARD ASSESSMENT: Determine nature of leg ulcer and exudate level Measure dimensions of area involved (i.e. LxWxD) DRESSING SELECTION: Determine dressing size needed. Allow for at least 1 to 1 1/4 in (3 cm) border around the visible wound area involved and select the appropriate size SureSkin II BORDER STANDARD dressing See Dressing Selection Guidelines page 12 DRESSING APPLICATION: 1) Clean wound area with normal saline or water 2) Pat area dry ( do not use ointments or moisturizers on skin) 3) With one hand, grasp paper release liner at end with arrows 4) Use other hand to grasp plastic film release liner at arrows 5) Position dressing over ulcer so that most of dressing is below ulcer 6) Remove paper release liner completely and smooth in place 7) Remove plastic film release liner completely and smooth in place 8) Place hand over dressing and mold dressing to skin contours 9) Apply sustained graduated compression wrap for venous ulcer WHEN TO CHANGE DRESSING: The compression wrap should be checked dailt and reapplied when needed. The dressing should be changed more frequently if drainage is heavy or if leakage occurs. DRESSING REMOVAL: 1) Remove compression wrap 2) With one hand gently lift edge of dressing from skin surface 3) Use other hand to press down on skin to separate dressing from skin 4) Use this technique to work your way around the dressing and loosen it completely from the skin 5) Gently roll dressing off skin 6) Alternatively, stretching the dressing horizontally and lifting it will also facilitate removal in most situations 7) Discard dressing and any packaging materials into appropriate receptacle NOTATION Do not use compression bandaging on Arterial or ulcers of mixed etiology without medical direction and supervision. Venous ulcers often emit an odor on dressing removal. Proper dressing disposal and wound cleansing help to minimize odor problem 10

14 Wound Dressing Selection GUIDELINES: To enhance dressing performance, it is recommended a minimum of a 1 to 11/4 inch (3 cm) border of intact skin remain around the wound, when choosing the appropriate size dressing to use. If the wounds fits in the circular diameter listed below use the dressing indicated Wound Size Dressing Size 0.5 inch 2 x 2 inch (1.25 cm) (5 x 5 cm) 2 inch 4 x 4 inch (5 cm) (10 x 10 cm) > 2-4 inch 6 x 6 inch (> 5.5 cm - 10 cm) (15 x 15 cm) >4 inch 8 x 8 inch (>10 cm) (20 x 20 cm) 12
