1 REGISTRATION REQUIREMENTS FOR THE 1 ST INDONESIAN INTERNATIONAL TATTOO CONVENTION ( IITACON ) The following outlines the registration requirements of the 1 ST International Tattoo Convention ( IITACON ) Indonesian DEMAND The venue where the convention is being held is limited in size. Approximately 100 booths are available and they will sell out quickly! The demand for booths each year has always exceeded the supply of booths available. Registration is open to body art shops and vendors worldwide. Registration is being delivered to over 500 shops and vendors. HOW TO ENSURE YOUR SPACE In order to maintain the highest level of quality for attending artists and collectors, booths at the 2012 show will be strictly limited to 100 shops and vendors. To secure your space at the 1 ST IITACON 2012, we recommend the following: 1. After receiving this form, complete and return it quickly by to registration@iitacon2012.com, making sure you fill out all forms accurately and in their entirety. 2. You ll then receive the booking confirmation from us with optional payment methods to pay the downpayment of your booking. 3. After you make the payment, send us the receipt to payment@iitacon2012.com. 4. You ll get the downpayment confirmation from us and the booth is booked for you. 5. 6 THE 1 ST INDONESIAN INTERNATIONAL TATTOO CONVENTION CONTRACT November 22 nd 24 th 2012, Bali, Indonesia VENDOR SET-UP Wednesday, November 21 st pm-06.30pm CONVENTION HOURS: Thursday, November 22 nd : 01:00pm- 10:00pm Friday, November 23 rd : 11:00am-11:00pm Saturday, 24 th : 11:00am 02:00am To All Booth Holders: Please Read Thoroughly Federation of Indonesian Tattoo as the Convention organizers in cooperation with Public Health of volunteers, released this guidelines to ensure a safe and healthy tattoo convention for all participants, staff, voulunteers, and the attending public, these guidelines are required for participation in the event. We are very fortunate to be working in conjunction with our local environmental health investigators. Please help us work within these guidelines to ensure that we may continue to sponsor our convention. Individual booths that do not comply with The 1 st Indonesian International Tattoo Convention (IITACON) Health and Safety Guidelines may be closed without any compensation or reimbursement of convention fees, travel expenses, or lost income. On Wednesday during setup, a health check in crew will be available and will run a checklist, and get you set up for inspections. Every booth that will be tattooing and piercing must go through this check in to be able to open on Thursday at 1:00 p.m. Please don t hold us up by being late or by not having what you need. INFORMATION, TERMS, AND GUIDELINES 1) -Vendor agrees not to move beyond the area designated to them by this contract, nor to block or obstruct any aisle, walkway or exit. An eight foot table will be provided to each vendor. Any special equipment, including extension cords, etc, is the responsibility of the party signing this agreement. 2) - All vendors MUST clean up after themselves and will be responsible for any and all damage they may cause to the venue. 3) - The 1 st Indonesian International Tattoo Convention is NOT responsible in any way for the merchandise sold at our Expo. Any legal disputes that may arise due to the services rendered merchandise sold, raffled or given away will be the sole responsibility of the vendor in question. By signing this agreement you agree that the 1 st Indonesian International Tattoo Convention is not liable for any damages, court costs and/or lawsuits that may result from your actions as a vendor at our Convention. 4) We respectively ask that vendors to be cooperative when displaying art and merchandise on their tables and/or wall space. There will be young adults present so we ask that all material featuring extreme nudity, culture insults and/or overtly violent images be kept out of sight or in discreet places. Violators will be removed from the Expo room, forfeit their vendor fee and will be banned for life from our events. 5) The 1 st Indonesian International Tattoo Convention, promoters, staff, volunteers and/or associates, are not responsible for any theft, damage or loss of vendor's property. While the Convention is in a very secured area and security will be present, vendors should not leave their table unattended. The Expo room will be locked after hours. 6) Vendors will set up and tear down their own tables. 7) No smoking is allowed in the Expo area. There are several areas directly outside the room where smoking IS allowed. 8) No refunds of any kind will be issued to vendors once space is reserved and paid for. 9) By signing this contract you agree to all of the above terms and requests and agree to be bound by them. Artist Requirements 1. All persons who have a reasonable anticipated risk for skin, eye, or mucous membrane contact with blood should comply with standard precautions/body substance isolation. 2. Federation Of Indonesian Tattoo will issue temporary practice license for The 1 st Indonesian International Tattoo Convention after the health check list inspection held by local environment health investigator. 3. Artists should not perform any tattoo or piercing procedures if they are ill or experiencing symptoms such as diarrhea, vomiting, jaundice, rash or open skin infections such as boils, impetigo, or scabies. 4. Artists must keep consent forms for each client. Additionally, all artists must provide aftercare instructions for each customer. Samples will be asked for at check in. 5. Artists must wash their hands before and after taking any breaks. Additionally, artists must wash their hands, or use an approved hand-washing alternative between clients. Please show us your alcohol wipes or antimicrobial hand wipes at check in.
7 Health requirements Invasive body art involves a high risk of transmission of blood-borne viruses such as hepatitis B and C and HIV, and bacterial infections that can be transmitted by unclean and non-sterile equipment and unhygienic procedures and premises. The potential for serious infection occurs during body art procedures because needles used to penetrate the skin become contaminated by blood and body fluids, which do not have to be visible on an instrument, needle or working surface for infection to be transmitted. There is also a risk of nerve damage and unwanted scarring if procedures are poorly performed. Every client and worker is at risk if proper infection control procedures are not followed. The client s skin should be clean and free of infection, and all instruments used in skin penetration practices must be sterile at the time of use. 1. Only single use disposable needles and grips/tubes shall be used and these should be stored in a suitable storage area for clean and sterile items to protect them from contamination. 2. Cover surfaces that may need to be touched (eg. Spray and ink bottles) with single-use plastic bags or plastic film so only the nozzles are exposed. Cover light fittings and power pack controls with plastic film. 3. Dispense the required pigment, lubricating jelly, antiseptic cream and any other lotion (including solutions used to clean the skin during the tattooing process into single use containers using single-use spatulas. 4. Place water for rinsing between colours into a single-use cup. 5. Place single-use wipes for one client in the area, wipes must be stored where they cannot become contaminated. 6. Open all sterile items (including tubes/grips and needles attached to needle bars) in the presence of the client to show sterile instruments are being used, check chemical indicators for colour change and, if satisfactory, then assemble the handpiece. Document any chemical indicator results on the client waiver form. 7. Any leftover lubricating jelly, pigments, creams, water and wipes must be discarded after each client. 8. Replace any sterile instruments or needles accidentally touched by the operator or contaminated in any way, either before or during a treatment, with another sterile instrument or needle. 9. Take care when inspecting needles for defects such as damaged or blunt points. Needles must never be tested for sharpness on the skin of the operator or client, self-illuminating magnifying glasses should be available to check needles for bluntness or barbs. 10. Each artist must use the sharps container supplied by the Organisers in their booth for the disposal of used needles, razors etc. and all contaminated waste, wipes, gloves etc must be disposed of in a separate contaminated wastes bin. 11. If an autoclave is used to sterilise equipment then a record must be kept of the length and time the equipment was sterilised and the temperature and the pressure of the autoclave. 12. Ensure the client s skin is clean and free from infection, sores or wounds on or around the tattoo site. 13. If the tattoo area needs to be shaved, use only a single-use safety razor for each client and immediately discard it after use into the sharps container. 14. Disinfect the site where the procedure will be carried out. 15. Use an antimicrobial lotion or plain liquid soap on the skin before placement of single-use stencil, multi-use deodorants must never be used. 16. Apply lubricating jelly to the tattoo site using a new single-use spatula for each client, if extra jelly is required, then use a new spatula, discard the spatula after each application, never use gloves or bare fingers. 17. Each artist must wash their hands in the separate hand basin using antimicrobial or plain liquid soap before and after attending each client, and if you have to leave the client during the procedure, Hands must be dried using a disposable paper towel and a new pair of single-use gloves must be put on. 18. A new pair of plastic disposable gloves must be worn on each hand for each client. The use of sterile single-use gloves is encouraged, the use of non-sterile single-use gloves is the minimum requirement. 19. Artists who are ill or have skin infections or sores must not perform a skin penetration procedure. Cuts must be covered by an appropriate waterproof bandaid. 20. Where possible, avoid contaminating the work area with the client s blood, avoid cross contamination between surfaces. 21. When tattooing, do not eat, drink or smoke. 22. If the client takes a break during the tattooing process, then cover the skin being tattooed with a dry clean dressing. 23. Use pre-dispensed cleaning solution and single-use wipes to remove excess pigment and blood from the tattoo site. Dispose of wipes into the clinical contaminated wastes bin. 24. When the tattoo is completed, clean the area with an anti-microbial solution, then remove gloves and wash and dry hands, then re-glove using new single-use gloves. 25. Apply antiseptic cream from a single-use container using a single-use spatula, cover the tattoo site with a sterile dressing or plastic film held firmly in place by surgical tape. Remove gloves and wash and dry hands again. 26. Take time to explain and demonstrate to the client how to care for the tattoo to prevent infection, and provide the client with the same information in writing, ensure the client has fully understood these instructions. 27. Each artist/booth manager must provide appropriate storage for any clean linen, towels and safety clothing they are using in the booth to prevent soiling and contamination, and a separate suitable storage area for staff personal items. General Requirements 1. Chairs, and surfaces must be non-absorbent, water impervious, and easily cleaned. 2. The tattooing of minors is prohibited, without parental consent. 3. All persons such as volunteers and vendors who have a reasonable anticipated risk for skin, eye, or mucous membrane contact with blood must comply with standard precautions/body substance isolation.
8 Applicant Statement of Consent I understand that in order to practice and body art at The 1 st Indonesian International Tattoo Convention I must have a valid license to practice in The 1 st Indonesian International Tattoo Convention issued by Federation of Indonesian Tattoo and that the license is only valid for the conduct of those body art practices for which I have applied, as listed on the license. I further understand that this license is valid only in The 1 st Indonesian International Tattoo Convention for the dates of November 22 nd, 23 rd, and 24 th I understand that if a temporary license to practice Body Arts in the 1 st Indonesian International Tattoo Convention is issued, that it is only valid for the convention location for those dates.
