Access Key label here This kit contains: SECURITY SEAL Paper Cup Sample Tube Security Seal Pipette Freepost Box Test Requisition Form Biohazard Bag Before you begin! Before taking your sample, make sure that you haven t passed urine in the last hour (This is because the test needs to use urine that has been present in your bladder for at least an hour) Unpack your test kit and use a ballpoint pen to clearly fill out your details on the Test Requisition Form (provided) Thoroughly wash your hands NOTE: Failure to complete the requested information will result in the laboratory being unable to process your sample. 1 Unscrew the Sample Tube lid and place the tube on a flat surface NOTE: The Sample Tube contains a very small amount of preservative fluid, please do not discard the fluid Use the disposable Paper Cup and Pipette to collect some urine and transfer it into the Sample Tube NOTE: Please ensure you collect the first bit of urine Please fill the tube to between the minimum and maximum marks Max Min Replace the Sample Tube lid Place the tube inside the Biohazard Bag NOTE: Make sure that the lid is secure and tight on the tube, otherwise your sample may leak in transit Peel off the silver seal adhesive. Apply pressure to seal the bag, working outwards to edges NOTE: Ensure there are no gaps in the seal
2 Attach Security 3 4 Place the Biohazard Bag and Test Requisition Form inside the Freepost Box NOTE: It is essential that you return the completed Test Requisition Form with your urine sample Ensure the Box is securely closed and the flaps are tucked inside the Box Attach the Security Seal to the flap and bottom of the Box, in the areas marked Post the package in your nearest post box Close the Box carefully Attach Security How to get your results What if I test positive? You will be notified by /sms as soon as your results are available online You can collect your results by logging on to the website: jbu-clinic.co.uk using the Unique ID (sent to you via /sms) and the Access Key from the sticker on the top right hand corner of these instructions Unique ID: ###### Access Key: ###### It is important not to worry if you test positive If you receive a positive result online, we will advise you on how to get treatment and advice It is also important to remember, any partners you have may also need treatment STOP! Access Key label here This kit contains: SECURITY SEAL Security Seal Pre-Printed Tube Label Swab Freepost Box Test Requisition Form Before you begin! Unpack your test kit and use a ballpoint pen to clearly fill out your details on the Test Requisition Form and Swab tube (provided) NOTE: Failure to complete the requested information will result in the laboratory being unable to process your sample. Thoroughly wash your hands 1 Remove the Swab from its tube. Avoid touching the tip of the Swab, or laying it down NOTE: Do not use if the seal is broken or damaged. Carefully insert the Swab half a finger length into your vagina, pointing the tip towards your lower back 3 4 Rotate the Swab a number of times around the inside of your vagina Return the Swab to the tube and ensure you tightly secure the cap Wash your hands Follow the instructions overleaf to return your sample to be tested
4 Attach Security 5 Attach the enclosed Pre-Printed Tube Label to the Swab tube Place the Swab tube and Test Requisition Form inside the Freepost Box NOTE: It is essential that you return the completed Test Requisition Form with your Swab. Close the Box carefully 6 Ensure the Box is securely closed and the flaps are tucked inside the Box Attach the Security Seal to the flap and bottom of the Box, in the areas marked Post the package in your nearest post box Attach Security How to get your results You will be notified by /sms as soon as your results are available online You can collect your results by logging on to the website: jbu-clinic.co.uk using the Unique ID (sent to you via /sms) and the Access Key from the sticker on the top right hand corner of these instructions Unique ID: ###### Access Key: ###### What if I test positive? 5 just between us Dry Blood Spot Collection Kit Before attempting to collect your sample, please read these instructions carefully. Access Key label here This kit contains: Important Tips Dry Blood Spot Card + Moist Wipes Pink Lancets IMPORTANT! The Lancet is spring loaded and will retract into the body of the device after skin puncture. Lancets can only be used once. The best fingers to collect blood from are the middle and ring fingers of your non-dominant hand. (i.e. if you are right-handed, collect your blood sample from fingers on your left hand) It is much easier to collect blood from warm hands. Alcotip Swab SECURITY SEAL Before you begin, make sure you have access to somewhere you can wash your hands, some clean tissues and a ballpoint pen. Alcotip Swabs Plasters Resealable Plastic Bags Test Requisition Form Security Seal Before you begin! Unpack your test kit and use a ballpoint pen to clearly fill out your details on the Test Requisition Form Fill out your name and the date on the Dry Blood Spot Card NOTE: Failure to complete the requested information will result in the laboratory being unable to process your sample. Please refer to our website to ensure your Dry Blood Spot Sample is performed correctly 1 Wash your hands in warm soapy water. (It is much easier to collect your sample if your hands are warm) Dry them thoroughly with a clean towel Using an Alcotip Swab (provided) wipe the selected fingertip Use a clean tissue to dry the finger tip Twist and remove the end of the Lancet (provided) The Lancet is ready for use 3 Take the Dry Blood Spot Card (provided) and open the upper flap to reveal the five circles Tuck Cover Here 4 Open the Moist Wipes (provided) The next step will draw blood Position the Lancet firmly against your finger tip and press the top Do not remove the Lancet from your finger until you have heard the device click. If you do not press hard enough or want another attempt, please repeat this step with a new Lancet (provided).
6 5 A small drop of blood will form on your fingertip. Using the Moist Wipes, wipe the first drop of blood away Pointing your hand downwards, massage the punctured finger to form of a new drop of blood Take your punctured finger and squeeze it gently to encourage the blood flow 6 Apply the blood directly onto the first circle on the Dry Blood Spot Card (provided), until the circle is filled with blood Repeat steps 5 & 6 until all five circles are full of blood NOTE: Please ensure that you fill all 5 circles to the dotted line, leaving no white areas inside the circle. If necessary, repeat step 4 with a new Lancet (provided). Correct: Incorrect: 7 Clean and dry any excess blood from your finger and cover the wound with the Plaster (provided) Leave your Dry Blood Spot Card to dry Tuck the cover of the Dry Blood Spot Card under the lower flap 8 Place any used and unused Lancets and the Dry Blood Spot Card into the resealable plastic bag (provided), and place into the Pre-addressed Box (provided) Double check that you have written your details on the Test Requisition Form and placed it inside the Box NOTE: It is essential that you return the Test Requisition Form with your blood sample. Seal the Box using the Security Seal (provided) How to get your results You will be notified by /sms as soon as your results are available online. You can collect your results by logging on to: jbu-clinic.co.uk using the Unique ID (sent via /sms) and the Access Key from the sticker on the top right corner of these instructions. Access Key label here This kit contains: Important Tips Test Requisition Form Alcotip Swab Biohazard Bag SECURITY SEAL Blue Lancets IMPORTANT! The Lancet is spring loaded and will retract into the body of the device after skin puncture. Fill the Blood Collection Tube to the upper line (500) on the side of the tube NOTE: If you are unable to collect enough blood, use the second Lancet on another finger.
8 Attach Security 5 Clean and dry any excess blood from your finger and cover the wound with the Plaster (provided) Once the lid is on, gently invert the collection tube 8 times Replace the lid of the Blood Collection Tube and screw on securely 6 Place the Blood Collection Tube as well as used and unused Lancets in the Biohazard Bag (provided) NOTE: Ensure there are no gaps in the seal. Peel off the silver seal adhesive. Apply pressure to seal the bag, working outwards to the edges 7 Inside the self-addressed, Freepost Box, please insert: How to get your results You will be notified by /sms as soon as your results are available online. - The Biohazard Bag containing the Blood Collection Tube & used Lancet(s) - Test Requisition Form Seal the Box using the Security Seal (provided) Remember: If you have requested additional tests, ensure all items are placed in the Freepost Box before sealing it NOTE: It is essential that you return the Test Requisition Form with your blood sample.
