Prior to drafting any contract for goods or services, the responsible party or the decedent s survivor who is handling the funeral arrangements is entitled to receive a copy of any agreement in the possession of the funeral establishment that has been signed and paid for, in full or in part, by or on behalf of the decedent. GENERAL PRICE LIST / SCHEDULE OF FEES The goods and services shown herein are those we provide to our customers. You may choose only those items you desire. However, any funeral arrangement you select will include a charge for our basic services and overhead. If legal or other requirements mean you must purchase any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. This list does not include prices for certain items that you may ask us to purchase for you, such as, cemetery services, flowers, and newspaper notices. The prices for those items will be shown on your statement describing the funeral services you selected. The only warranties granted in connection with the merchandise sold with this funeral service, are the express written warranties, if any, extended by the manufacturers thereof. No other warranties of either merchantability or fitness for a particular purpose are extended by Chapel of the Valley Mortuary. BASIC SERVICES OF THE FUNERAL DIRECTOR AND STAFF... $ 2, This is a basic fee that we charge every family for our professional expertise in assisting them with their preferences. Our charge includes, but is not limited to, 24 hour availability of staff to respond to initial request for service; consultation and coordination with family or other responsible parties for the arrangement of desired services; preparation and filing of the death certificate and any other necessary authorizations and permits (consulate fees not included); recording of the vital statistics; preparation and placement of obituary notices, consultation and coordination with related agencies such as, clergy, cemetery, crematory, common carrier, vault companies, insurance companies and any others as requested or required. Also included in this fee is: overhead expenses relative to our facilities such as, professional licensing, accounting fees, insurance, building, utility and maintenance costs, taxes, and administrative and State/Federal compliance expenses. This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. This fee is already included in our charges for direct cremations, immediate burial, and forwarding or receiving remains. TRANSFER OF DECEASED TO MORTUARY WITHIN LOCAL AREA*...$ *Local area includes Antelope Valley and surrounding communities. Other areas will be determined by rate schedule. PROFESSIONAL EMBALMING...$ Except in certain cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. COSMETICS, DRESSING OF DECEASED AND CASKETING...$ ADDITIONAL CARE OR PREPARATION OF THE DECEASED 1.
