1 2- Day Course Preparation Details D1 Cosmetic/Therapeutic, D2 Advanced, D3 Business, D4 Lips & PRP Staff Attendance for D1, D2, D4: If you wish to bring a staff member to attend the course with you both days, you may bring one person for $425, including breakfast & lunch both days. Breakfast & Lunch is provided both days. Please let us know in advance if you have any special food requirements such as allergies or vegetarians. Attire: Day 1 Lecture: Business Casual Day 2 Clinical: Scrubs, lab coats (what you usually wear at your practice) Airports for Miami Courses: Miami International (MIA) is about 20 miles from the teaching facilities Ft Lauderdale (FLL) is about 12 miles from the teaching facilities. Model Patients for D1, D2, D4: The Day 2 Hands- on clinical will consist of treating live model patients in a dental practice. You can choose to either bring a model patient, or we can provide a volunteer model for you at no extra charge. All models are pre- qualified and proper follow- up is done with each patient post- treatment. If you would like us to provide a model for you, please let us know within 2 weeks of the seminar date. We will be sending you pre- treatment instructions that you will need to provide to your patients at least 48 hours prior to the day of treatment. If we are providing a patient for you, we will be providing your patient this information. We will also be providing this information during the course, however, it is important they also receive it prior in preparation as may patients are on daily medications and/or supplements that may make them NOT ideal patients for injections. Patients who bruise easily because of medications that act as blood thinners or other disease processes may not be good candidates. Even though treatments with Botox and dermal fillers are minimally invasive treatments, patients should know that bruising and swelling is possible and is normal post- treatment. Model patients should not have any important work or personal events coming up within two weeks of treatment. Day 2 is the only day you will need a model patient. Your patients should arrive at 8:30 AM to fill out consent and medical history forms and take before pictures. Because this is a learning environment for the doctors, we cannot designate specific treatment times so patients should prepare to be available for the day (unless otherwise discussed). Any time restraints will be accommodated as best as possible.

2 DentaSpa 1: Therapeutic/Cosmetic Botox & Filler Dental Training Please respond to seminars.com with the answers to the following 2 questions: 1. Dermal Filler Product Need (1 syringe of Juvederm, $257 or 2 Syringes (1 Box, $515) 2. Patient Need - Will you be bringing someone (Patient Name) or would you likeus to provide a volunteer model for you for Botox and/or Fillers? Day 1: Saturday, 9AM 5PM: Comprehensive Botox & Dermal Filler Lectures, Marketing/Patient Education, Case Presentations Day 2: Sunday, 9AM 5PM: Full- day Hands- on Clinical Participation Treating Model Patients When selecting your model patient(s), this is what you are looking for: For D1 THERAPEUTIC: BOTOX Treatments: Patients who suffer from facial pain, TMD, clenching, gummy smiles and smile asymmetry. We will also be treating frown lines and forehead lines for facial pain and migraines stemming from TMD. For Dermal Filler Treatments: Patients who need minimal perioral improvement, due to volume loss caused by aging. Dermal fillers will be used to help with or prevent angular cheilitis, aid with retention of removable prosthodontics and address post- orthodontic volume loss. Treated areas may include lips, commissures, marionette area/lines, nasolabial folds. Also consider patients with gummy smiles and those who need class II profile correction and class III profile correction. For D1 COMPREHENSIVE: Botox Treatments: Someone who would like to treat forehead lines (lines made when patient raises their eyebrows to make a surprised expression), frown lines (vertical lines between the eyebrows know as glabellar lines) formed when the patient makes a mad expression and/or crow's feet (wrinkles around the corner of the eyes caused from squinting). If a patient has a gummy smile, we will also be treating him/her with Botox. Dermal Filler Treatments: Someone who would like to treat their nasolabial folds (lines from the corner of the mouth to the corner of the nose), marionette lines (lines from the corner of the lips that go down the face toward the jaw line), and the corners of the lips to bring up smile line. Lip augmentation may also be done on a patient that would like to enhance the fullness of their lips. The best candidates are patients with light to moderate lines because you will more likely get a full correction.

For your training convenience, we order the clinical product for the class and provide it at cost. During the class you will learn how to set up your own accounts to place future orders at your office. For the clinical, most participants purchase 1 box (2 syringes, $515). This will allow you to do a comprehensive case with more injection practice. You may select to just get one syringe, $257. Please let me know what I should order for you. DentaSpa 2: Advanced Botox & Dermal Filler Dental Training Please respond to ana@dentaspa- seminars.com with the answers to the following questions: 1. Will you be bringing a patient or would you like me to provide a volunteer model for you? 2. Will you be partnering? This lowers dermal filler costs for clinical 3. Have you sent back the D2 Evaluation Form (provided to you)? Once we receive this back, along with knowing if you want to partner, we will let you know the dermal filler product you will need. Day 1: Saturday, Lectures: 9AM 5PM We will have continental breakfast for everyone starting at 8:30. During the morning we will be covering the dermal fillers lecture followed by video demos and interactive class treatment planning. After lunch, we will hold the dental therapeutic presentation and business presentation. We will conclude the lecture day with the Botox presentation followed by video demos and interactive class treatment planning. Day 2: Sunday, Hands- on Clinical 9AM 5PM Sunday will be hands- on all day and this will be the only day you will need a patient(s). It is highly recommended that you send in a pictures for your patient to ensure they are candidates for treatments. Prior to treatment, all patients will fill

4 out and sign Botox and dermal filler informed consents forms, medical history forms, and have their before pictures taken. When selecting your model patient for the D2 Advanced course, this is what you are looking for: Botox Treatments: Someone who is 40+ who wants overall facial contouring with Botox including treatment of DAO, neck, eyes, pebbled chin, commissures, gummy smiles and other areas. In addition to combination Botox/filler treatments to enhance treatments, participants are also going to also be doing therapeutic treatments with Botox including TMD, gummy smiles, tension headaches, facial migranes and learn where to inject for masseter hypertrophy treatment (patients for this can be any age, 18 and over). If model patients do not have certain therapeutic needs, instructors will still be teaching lecture and demonstrating injection points on how to provide those treatments. Dermal Filler Treatments: Someone who is preferably 40+ who wants overall facial volumizing including areas such as chin, cheeks, jaw, jowls, and lip augmentation with combination treatment.. Best candidates are hoping to restore facial volume lost through aging. Depending on your experience level, you may want to review some of the areas covered in D1. If your patient has some of the problem areas mentioned above, they will more than likely also benefit from the areas covered in DentaSpa 1. During the clinical day, you can diagnose all areas for learning purposes and decide with the trainer what areas to treat for your optimal learning experience and benefit of the patient. We will be sending you pre- treatment instructions that you need to provide to your patients at least 48 hours prior to the day of treatment. Again, all patients must be approved to be part of the D2 clinical by submitting a non- smiling photo. If we are providing a patient for you, we will be providing your patient pre treatment information. Included with Registration: Up to $400 in Botox units to use during Sunday's clinical Complete Botox & Filler Lecture Presentations and Training Materials Before/After pictures of your model patient(s) Certificate of course completion (16 CE credits) Breakfast & Lunch included both days Product for Clinical - Additional Costs: After receiving your D2 evaluation and request to partner or not partner, we will send you the recommended product you will need to complete your case. While we have additional product if needed, we will need to order at least your base product

Partnering for Clinical: Because we are doing more extreme makeovers on patients, participants will be doing several Botox and dermal filler treatments that require a lot of product. For this reason, we give participants the option to work with a partner so they can do all the procedures but share costs of product with a partner participant. D3: Master the Business of Botox & Fillers in Dentistry 16CE Day 1: Comprehensive Lectures & Presentations: 9 AM - 5 PM A full day of exciting and educational lectures on various aspects of implementing BOTOX, dermal fillers and other profitable related treatments into the dental practice. Topics include staff training, strategic marketing, branding, patient referral programs, delivering promotions and much more. Experts - including successful dentists in the industry, and business consultants- will be sharing valuable business knowledge, success pearls, marketing tools, and discussing different products, technologies and business strategies for continued business growth and success. Day 2: Customized Learning Stations: Open from 8:30AM 6PM (at your discretion) Participants will enjoy a day of 1:1 consulting opportunities at various learning stations throughout the day, with stations including a team training session where doctors and staff members can learn and review the A- Z practice training manual on BOTOX and Dermal Filler implementation. Another valuable station allows a customized session with an expert business consultant to discuss goals and ideas for successful business growth with personnel from each attending practice. At various aesthetic stations, participants will also be exposed to several technologies and products that can be coupled with injectables to provide a significant boost in revenue, requiring no additional chair time. We encourage that participants bring staff members who will be involved in developing a successful BOTOX and Dermal Fillers business venture within your dental practice. Participants will receive an 80 + page A- Z Aesthetics Business Training Manual to help the entire dental team successfully implement treatments. Because of liability with non- FDA and/or non- USA purchased product, participants are required to purchase dermal filler product at cost for treatments provided during clinical trainings. PRP Clinical: Patient: Will you need a patient? If you are bringing someone, what is your patient's name? Product Needed: PRP Kit: $99. Would you like me to charge the same card on file? Day 1: Saturday, Lectures 9AM 5PM Saturday we will be holding the lecture component of the program from 9am - 5pm. We will be covering lip augmentation in depth including techniques, product options, classification systems, case reviews, video demos and live demonstrations. We will also cover the use of PRP for aesthetics. *It is recommended that you submit a lip augmentation case you may have done so the trainers can review this case with you and the class. Day 2: Sunday, Hands- on Clinical, 9AM 5PM

7 Sunday will be all day hands- on and this will be the only day you will need to bring your patient(s). All participants will do a lip augmentation case and if requested, a PRP case. We will have various products available that can be purchased during the class depending on the diagnosis of your patient. Lip Augmentation Model Patients: When selecting your model patient or patients, this is what you are looking for: Someone who would like to enhance or add overall volume to their lips Someone who has lip lines or asymmetries to their lips Someone who has lost the border of their lips or would like to establish more definition to this border Someone who has dropped corners of their mouth
