ARTIST INFORMATION Fellow Artist, Thank you for your submission(s) to the 6 th Annual Swedish Multiple Sclerosis Center Art Show. We are delighted to accept your art to this year s show. Please invite your family, friends, and neighbors to see your work on display, Saturday and Sunday, June 27 and 28 at the Seattle Center Armory. To prepare for the show, please read the following important information. ART WORK CRITERIA ALL artwork will be accepted and displayed ALL art abilities are welcome We accept art from anyone affected by MS (patients, family, caregivers, friends, coworkers and children) One DOES NOT have to be a patient of Swedish Medical Center to participate ALL art mediums are welcome Limit size of work to 36 x 36 Two art entries per person All submissions must be approved by the MS Center Art Show Committee prior to display PREPARING YOUR ART All art must be packed and ready for display (framed, labeled, appropriate hanging materials attached to frame) before the show. Your art WILL NOT be displayed if it does not have the appropriate display materials. Label each entry with the following information: Artist s name Title of art piece Sale price (if applicable) Phone number Email address Protect your art work with wrapping. Possible ideas: Bubble wrap Box Bag Cloth or towel BE SURE TO LABEL BOTH THE ART WORK AND THE WRAPPING
ARTIST INFORMATION CONTINUED ART WORK DROP OFF INSTRUCTIONS You may drop off your art work any weekday up until June 1 Staff will be available between 9:00 a.m. 4:00 p.m. to receive art work Submit your art piece to the reception desk at the MS Center MS Center at Swedish Jefferson Tower at Swedish Cherry Hill Campus 1600 East Jefferson Street, A Level Seattle, WA 98122 All artwork will be professionally transported and installed at the Seattle Center. ARTIST INFORMATION CONTINUED ARTIST MEET, GREET AND CAKE Join us for an artist meet and greet at the exhibit Sunday, June 28, 5:00 6:00 p.m. This will be an informal gathering celebrating the contributions and creativity of each artist. Family, friends, and caregivers are welcome. Enjoy a complimentary piece of cake on behalf of the Swedish MS Center! ART WORK PICK UP INSTRUCTIONS There are two options for picking up your art: 1. Pick up your art from Seattle Center from 6:00 p.m. 7:00 p.m. on Sunday, June 28 (after artist meet and greet). 2. Pick up your art from the Swedish MS Center after 2:00 p.m. Monday, June 29, or throughout the month of July between 9:00 a.m. and 4:00 p.m. Bring photo identification when picking up art work. Staff may not release the piece(s) unless you, or an authorized representative bring photo ID of your driver s license and/or signed consent for release. Art not picked up by July 31 will be donated to charity or become property of the Swedish MS Center. Please contact us ahead of time if you need special accommodations.
ARTIST WAIVER AND RELEASE I,, will be displaying my art for the Swedish MS Center Art Show to be held at Seattle Center Armory on Saturday, June 27 Sunday, June 28, 2015. I understand, acknowledge and agree to the following: My art is ready to be hung or placed and is appropriately labeled and framed (if applicable) and includes all needed hooks, wires and /or pedestals. Swedish Medical Center and the MS Center will NOT be liable to me or my agents for any harm or injuries suffered by me or my agents relating to placement and/or take down of my art at the Swedish MS Center Art Show. Swedish and the MS Center do NOT provide insurance for the loss, damage, or theft of my art while it is being displayed at the Show and will NOT be held liable for any loss, damage, or theft caused during the Show or during transit to and from the Show and while the art is stored at Swedish MS Center. I hereby release, discharge and agree to hold harmless Swedish, MS Center, its employees, agents and individuals involved in creating the Show from and against all liability as a result of any harm, damage, and/or loss of my art. I agree to defend, indemnify and hold harmless Swedish, MS Center, its employees, agents and individuals involved in creating the Show from any claims by third parties alleging that my art infringes on any third party s rights. I have read this waiver and release and by signing below, I warrant that I fully understand the contents of this waiver and release.
