Metro Hearing Aid The cost is associated with the repair or purchase of a hearing aid appliance and is in accordance with rate Hearing Aids established between the Resident and the company at the time of transaction. No fee for a hearing assessment. 11. Denture Clinic Services The cost for this fee for use service is in accordance with the rate established and agreed upon between the Resident and the company at the time of transaction. 12. Dental Services The cost for this fee for use service is in accordance with the rate established and agreed upon between the Resident and the company at the time of transaction. 13. Eye Care Services The cost is associated with repair, replace or purchase glasses or other eye apparel and this fee for use service is in accordance with the rate established and agreed upon between the Resident and the company at the time of transaction. 14. Mobility and Assistive Devices Assessment The cost to have a mobility assessment completed is approximately $135. 15. Escort Service Escort Services are provided at a Minimum 4 hours (hourly rate x 4 hours) plus cost cab fare. Statutory and Civic Holidays are billed at time and a half.
