APPENDIX D Monitoring Well Abandonment Correspondence with NJDEP: July 13, 2016 Memo to NJDEP Re: Recommendations Regarding Monitoring Well Decommissioning April 26, 2016 Letter to NJDEP Re: Request for Approval to Decommission per N.J.A.C. 7:9D-3.1(b) & (g) NJDEP Well Permit No. 2600003455 May 17, 2016 Letter to NJDEP Re: Well Decommissioning Authorization Request

Results you can rely on TRC 41 Spring Street New Providence, NJ 07974 Main 908-988-1700 Fax 908-464-3712 Memorandum To: Donna Gaffigan and David Morrow New Jersey Department of Environmental Protection From: Art Goeller, LSRP Subject: Recommendations Regarding Monitoring Well Decommissioning Date: July 13, 2016 CC: Chandra Patel (Roche), Greg Cierpial (Roche), Teresa O Meara (Roche), Walter Fedick (Roche), Dawn Pompeo (TRC), Dan Nachman (TRC), Linda Caramichael (TRC), Paul Furtick (TRC), Troy Gill (TRC) Project No.: 198233.0005 BACKGROUND Following the Hoffmann-La Roche, Inc. (Roche) Nutley soil remediation and facilities decommissioning activities completed throughout the Nutley campus, TRC identified 95 monitoring wells that had experienced some damage, mostly to the flush mount components (manways, pads). TRC evaluated the wells and determined that 19 of the damaged wells are not needed for ongoing Interim Remedial Measure (IRM) or long-term groundwater monitoring, and therefore should be decommissioned (i.e., formally abandoned by a New Jersey-licensed well driller). TRC proposes to have each well sealed for its entire open interval (or a variance from N.J.A.C. 7:9D will be obtained), and (in accordance with N.J.A.C. 7:9D), obstructions (if present) will be removed or drilled out prior to well sealing. The wells proposed for decommissioning, including decommissioning rationale, are listed in Table 1, attached, and their locations are shown on Figure 1, attached. Roche requests NJDEP concurrence that these 20 wells can be abandoned. ENVIRONMENTAL ENERGY INFRASTRUCTURE
