Our goal is to support you with a successful introduction to JUARA that will delight your customers and grow your business. From our generous and ongoing provision of collateral, to our training program, our dedicated JUARA team is here to support you and your business. Perks of a JUARA Partnership Samples, Testers and Brochures A simple order point system makes it easy to customize your needs for samples, testers and brochures for your opening order and all re-orders. Loyalty Rewards Customers who order at a minimum frequency and volume per year are rewarded special rewards and perks (see below) Training Training is scheduled preferably within 2 weeks of your receiving the opening order. If a face-to-face training cannot be done due to geographical reasons, a phone training will be scheduled with the lead staff of the spa and/ or boutique. Spa Treatment Protocol We provide a special treatment protocol with JUARA products with cost per treatment details, which will help you develop JUARA treatments as part of your spa menu. Web Locator Spa/Store address and phone number will be placed on JUARA s webpage, searchable by State. Full-sized testers Comprehensive training manual Attractive samples E-mail Blasts For qualifying locations, JUARA will notify its subscribers of a Featured Spa/ Featured Store with a localized E-mail blast. Trade Specials Monthly trade specials are announced by e-mail and include Gift-with-Purchase, trade discounts or free products with minimum purchase. Order Minimums Order Point System Our simple order point system makes it easy for you to pick and choose samples and testers you need according to the dollar amount you order from JUARA. For your opening order, you receive 2 points for every $100 ordered. For your reorder, you receive 1 point for every $100 ordered. These points will buy you the samples and testers you need. You can choose from the following samples and testers Testers Each full-size tester costs 1 order point. Minimum Opening Order: $ 500 Minimum Re-Order: $ 100 Samples Rice Facial Cleanser Casepack contains 20 packette samples of 0.14 oz/ 4 ml each Each casepack costs 1 order point. Opening Order Get 2 points for every $ 100 ordered Example If your opening order is $1,000 before shipping and handling, you earn 20 opening order points. Re-Orders Get 1 point for every $ 100 ordered Example If your re-order is $400 before shipping and handling, you earn 4 opening order points. JUARA Loyalty Program We look forward to building a long-term relationship with you and growing the business together. Our Loyalty Points Program is designed to reward partners that order consistently throughout the year and reach a certain annual order volume (not including shipping & handling). The more you order over the course of a year, the greater the pay-off at the end of each Calendar year. Platinum Partner $ 6,500 or more in annual orders Receive a credit of 5% of total annual orders at the end of the Calendar year. Gold Partner $ 5,000 to $ 6,499 in annual orders Receive a credit of 4% of total annual orders at the end of the Calendar year. Silver Partner $ 3,500 to $ 4,999 in annual orders Receive a credit of 3% of total annual orders at the end of the Calendar year. Bronze Partner $ 2,000 to $ 3,499 in annual orders Receive a credit of 2% of total annual orders at the end of the Calendar year. Minimum annual order volume is calculated before shipping & handling and after returns. Your orders are automatically tallied up throughout the year (not including shipping & handling). On January 1 st, your account resets to zero to start collecting loyalty points for the new Calendar year. To maintain your points throughout the year, we ask that you place a minimum $200 reorder at least once per every 3-month period. If more than 3 months pass between orders or if you order less than $200 in a quarter, your account will reset to zero. If you qualify to receive a year-end credit, it is applicable toward your first order in the new calendar year. JUARA reserves the right to change the requirements of the partner level rewards at any time. 11

Re-Order Please fax your completed Order Form to 646-365-3357, or e-mail it directly to orders@juaraskincare.com Payment Terms Up-front payment is required for all opening orders and first 3 reorders. Payment by credit card or check are possible. Terms of Net30 can be applied if the first 3 reorders are placed within 6 months of placing an opening order. How long will it take for an order to ship? All JUARA products are manufactured in the USA and ship out of our fulfillment warehouse in Massachusetts. Orders received later than 4 PM EST will not be processed until the following business day. 1) Opening Order Opening orders will ship within 5 business days of the time that the order and completed paperwork are received. 2) Re-Order Re-Orders will ship within 3 business days of the time that the re-order has been received. How are orders shipped? Orders are shipped by UPS Ground unless requested otherwise. Expedited shipping can be requested at an extra shipping charge. For shipments to the East Coast, it takes approximately 1-2 business days. Shipments to the West Coast usually take 5 business days. All orders incur a shipping and handling charge. Our shipping and handling fee is priced at cost, i.e. we just pass the actual shipping and handling cost on to the customer. We do not mark up the shipping and handling charge for profit. 12
