Ordering Information 4 Order Information Please quote your account number when placing an order. Please quote model numbers and any other specification required when ordering. Footwear - in order for us to process your order successfully we need you to provide accurate information. You will find width fittings on the measurement charts and item numbers and names with each product. Anti Varus versions are Reverse Lasted and marked in the catalogue as such. Please quote style, colour, material, size, width, Anti Varus/Over Splint (if required) and fastening arrangement e.g. velcro/lace/hook and lace/velcro. Orders placed by telephone then faxed or posted should be clearly marked with CONFIRMATION ORDER to avoid risk of duplication. FitzKidz products are sold in the Republic of Ireland by ATG Ltd, a company registered in the Republic of Ireland. FitzKidz products are sold in the UK by Ortho Europe Ltd who participate in the NHS PURCHASING CARD SCHEME and can accept orders / payment by this method. Response to Orders We maintain large stocks of products to ensure prompt delivery. Orders for stock items are usually processed and despatched the same day. All stock items will be despatched within 2 working days. All stock items requiring adaptions and Little Wonder Inlays will be despatched within 3 working days. Special orders such as Anti Varus, Over Splint and split sizes are within 21 working days. Carriage will be charged extra, at cost. Incorrect Delivery On receipt of the goods the customer is required to check the consignment. In the unlikely event that the type or quantity of goods is incorrect then you are requested to notify our Customer Care Department within seven working days of the date of delivery. After seven working days FitzKidz cannot accept responsibility for an alleged incorrect delivery. Return of Goods Policy Authorisation must be issued by FitzKidz / Ortho Europe prior to goods being returned, please contact Customer Care for a return reference number and ensure sections 1-5 of Return Advice Note (on the back of the despatch note) are fully completed. Ortho Europe will only accept goods for return that are as new. Soiled, damaged, modified or obsolete goods and goods made to special order may not be returned. Any goods returned must be done so within 28 days of the original delivery date. Goods must be returned in their original packaging. Split size boots will incur a 25% restocking charge. Anti Varus boots are nonreturnable. Single Patient Use It is a condition of Sale that these orthopaedic products purchased from FitzKidz or Ortho Europe is restricted to Single use Only by the originally-fitted patient in order to protect the care provider and the patient against potentially adverse consequences of infectious disease transmission or material instability resulting from previous use by the original wearer. Any expressed or implied warranties are voided if the orthosis is re-used for another patient. Any orthosis purchased must be prescribed and applied by a qualified clinician to ensure that it meets the needs of the particular patient and accomplishes the desired results. As a result, we have developed and brought together a range of footwear which matches the needs of both clinicians and patients alike by building on successful designs and approaches, using the skills and expertise of the manufacturers and benefiting from the diligence of clinical staff. With the additional assistance and suggestions from other fellow professionals, we can now offer children and young adults a range that suits their everyday needs and gives paediatric clinicians the opportunity to provide excellent quality footwear which looks good too. The colour range and materials are designed to suit a wide spectrum of activities for boys and girls and offer a first class choice in both school and leisure wear. We now offer an increased choice of Anti Varus boots with a standard 21 day delivery. There are High Stability boots available from stock in 48 hours and there are also extended options of Over Splint boots with some styles being offered off the shelf and others for delivery removable contoured inlays adaptable/ replaceable sole units within 21 day option. All footwear are available in split sizes within their size range. An additional 25% supplement is added to these and the Anti Varus range. The whole range is available with optional Velcro or Hook and Eye arrangement except High Stability Boots, they have a combination of Lace and Velcro for optimum control. We have chosen four width fittings, all of which easily accept shell orthotics and functional foot orthoses simply by removing the full-length moulded inlay. The sole units are hard wearing with flexible foreparts and are designed to accept modifications such as sockets and raises. All FitzKidz footwear models are also fully adaptable by our in-house adaptation service where sheepskin lining, wedges and many other features can be requested when you order. All adaptations are carried out by firstclass technicians using high quality components. Where patients have different needs for each leg, ranges can be combined to suit. For example if your patient wears an AFO splint, one oversplint boot and one standard boot could be ordered to allow for this. (In limited colours only please ask for details). Ordering Information Order Information Please quote your account number when placing an order. Please quote model numbers and any other specification required when ordering. Footwear - in order for us to process your order successfully we need you to provide accurate information. You will find width fittings on the measurement charts and item numbers and names with each product. Anti Varus versions are Reverse Lasted and marked in the catalogue as such. Please quote style, colour, material, size, width, Anti Varus/Over Splint (if required) and fastening arrangement e.g. velcro/lace/hook and lace/velcro. Orders placed by telephone then faxed or posted should be clearly marked with CONFIRMATION ORDER to avoid risk of duplication. FitzKidz products are sold in the Republic of Ireland by ATG Ltd, a company registered in the Republic of Ireland. FitzKidz products are sold in the UK by Ortho Europe Ltd who participate in the NHS PURCHASING CARD SCHEME and can accept orders / payment by this method. Response to Orders We maintain large stocks of products to ensure prompt delivery. Orders for stock items are usually processed and despatched the same day. All stock items will be despatched within 2 working days. All stock items requiring adaptions and Little Wonder Inlays will be despatched within 3 working days. Incorrect Delivery On receipt of the goods the customer is required to check the consignment. In the unlikely event that the type or quantity of goods is incorrect then you are requested to notify our Customer Care Department within seven working days of the date of delivery. After seven working days FitzKidz cannot accept responsibility for an alleged incorrect delivery. Return of Goods Policy Authorisation must be issued by FitzKidz / Ortho Europe prior to goods being returned, please contact Customer Care for a return reference number and ensure sections 1-5 of Return Advice Note (on the back of the despatch note) are fully completed. Ortho Europe will only accept goods for return that are as new. Soiled, damaged, modified or obsolete goods and goods made to special order may not be returned. Any goods returned must be done so within 28 days of the original delivery date. Goods must be returned in their original packaging. Split size boots will incur a 25% restocking charge. Anti Varus boots are nonreturnable. Single Patient Use It is a condition of Sale that these orthopaedic products purchased from FitzKidz or Ortho Europe is restricted to Single use Only by the originally-fitted patient in order to protect the care provider and the patient against potentially adverse consequences of infectious disease transmission or material instability resulting from previous use by the original wearer. Any expressed or implied warranties are voided if the orthosis is re-used for another patient. Any orthosis purchased must be prescribed and applied by a qualified clinician to ensure that it meets the needs of the particular patient and accomplishes the desired results.
