Bronwyn Williams turns to the experts to discover what's new, now and coming soon to a clinic near you. LESS IS MORE Going under the knife (or needle, for that matter) no longer means plastic-looking results. Advances on many fronts allow surgeons to work with a lighter touch and, coupled with a shift in patients' goals, results are now more about improvement than alteration. "People want to look like themselves, just well-rested, rejuvenated and good for their age, says Dr Sarah Hart, a specialist in advanced injectable treatments at Skin Institute. HOLISTIC APPROACH The way professionals approach a patient is changing too. "Rather than looking at a single area as a problem, there's a much bigger focus on the frame [the face shape] and the canvas [the skin], in a holistic approach to treatments, says Emma Lindley, Skin Institute appearance medicine trainer and nurse. Stacey Power, a cosmetic nurse at Ever Young, works with the principles of mathematics to create the perfect face. Using the golden ratio of the ancient Greeks, Power marks out minute measurements to guide her needlework, addinga little filler to lengthen a chin, or a bit of Botox to drop a brow, resulting in harmonious proportions. ON TREND Botox and fillers are by far the most common non-surgical procedures, although there is also a trend towards usingyour own biological products. "More people are aware of what's being put into their bodies so techniques which use the skin's own healing mechanism will continue to be in demand," says Marianna Glucina of About Face. BEAUTIFUL TECHNOLOGY New technology developments are allowing specialists to deliver results with significantly less recovery time. Lasers have come a long way from their original rough and ready beginnings and are being used fora variety of procedures, from mole and vein removal (performed at the Palm Clinic), to tattoo and hair removal, and skin rejuvenation. The use of technology is also gaining momentum in the field of cosmetic surgery. At Tauranga's Da Vinci Clinic, a 3D Vectra imaging camera shows patients what they'll look like post-surgery. Technology is aiding skin examination too. The Face Place photographs skin under UV light to reveal sun damage and pigmentation, and elsewhere gaining traction as an in-depth examination tool. Smith's sight is set farther into the future. "Our team is starting to think about big questions like: 'How can we use technology like virtual reality to better train our people and make great skin more available to every woman? How is artificial intelligence going to help us diagnose skin issues more effectively and provide better solutions based on evidence rather than opinion?'"

Under local anaesthetic, excess skin is removed, lifting and opening the eye and reducing dark circles. In place of a surgical facelift, elasticated threads can be inserted into the face, offering a vastly less invasive option. "This new generation of threads is without the barbs of the past, so they don't snap, cannot be seen, they build collagen, and, I am told, last forever," says Power. A variation on the theme, tissue screws, which provide augmentation and lift, are also on their way, says Dr Tristan de Chalain, a plastic and reconstructive surgeon at Auckland Plastic Surgical Centre. An increased focus on skin texture has given rise to a number of non-surgical improvement treatments. For dull, scaly skin, microdermaplaning ($150 at The Face Place) has taken over as a safer alternative to microdermabrasion. Using a fine plane, much like a razor, the treatment removes dead skin cells and 'peach fuzz' hair for a renewed smooth, silky texture. For large pores and fine lines, Ever Young's MicroBotox (from $1200 with HA) is the answer. "Botox is injected with tiny needles superficially into the skin to treat the minuscule muscles of the pores and skin surface," says Power. "It relaxes the muscles, tightening the surface of the skin." When mixed with a fine hyaluronic acid (HA) filler this is further enhanced, giving a silky, airbrush finish. For skin rejuvenation without downtime, The Face Place's Dermafrac (from $280) delivers customised serums using micro-needling to improve product absorption, paired with suction to draw more blood to the skin. Serum infusions range from hydrating HA to rejuvenating growth factors and vitamin C for skin lightening. 1 Whether you re looking/or structural enhancement over the cheekbones, soft plumping of the lips or a pinpointfocus in your smile lines, treatments can now take a tailored approach" NEW WAYS WITH BOTOX New off-label uses for the product, range from treating excessive sweating to fixing gummy smiles. Teeth grinding is common and can result in increased muscle mass around the jaw, giving the face a square look. With Botox, not only does the grinding stop, but over time the face slims down. The Face Place (from $360) and Ever Young (from $525) both offer this treatment. A recent study in the Journal of Psychiatric Research showed that 52 per cent of patients had relief from symptoms of depression after Botox was injected between the brows. The theory is that our facial expressions influence how we feel. By discouraging frowning, Botox makes patients feel less blue. Allergan (the company that makes Botox) is now running clinical trials, aiming for the USFDA to approve it as an official treatment for depression. DESIGNER FILLERS As fillers increase in popularity, product varieties are also expanding, giving doctors an ever-expanding toolkit in which to mould the face without surgery. Whether you're looking for structural enhancement over the cheekbones, soft plumping of the lips or a pinpoint focus in your smile lines, treatments can now take a tailored approach. There are duration options too, with short-acting (three to six months), medium-term (12-18 months) and permanent fillers, although de Chalain recommends avoiding the latter. The safest long-term fillers, he says, are your own fat cells, harvested in a fat transfer surgical procedure (priced from $2,100). Under local anaesthetic, fat is taken from the abdomen or thigh and placed into the face. Standard fillers are also being used for liquid rhinoplasty (from $790 at The Palm Clinic), a nonsurgical nose job in which product is injected to correct, smooth or balance asymmetry. 64 simplyyou.co.ii/

"Labiaplasty has become increasingly popular, both for aesthetic and comfort reasons," says de Chalain, who now does up to two surgeries a week. The procedure takes just over an hour, is done under local anaesthetic and prices start at $4,400. With genital procedures becoming increasingly popular, people are also looking for non-surgical alternatives. "Internationally, fillers designed specifically for the genital area are becoming more prevalent, although they are not yet readily available here," says Stone. "They can be used to plump out the labia majora [outer lips] which tend to shrink as we get older, creating a more youthful appearance to our most intimate parts." Balding or thinning hair is a pain point for many women and up until now few treatments offered immediate, long lasting results. Hairstetics is an artificial implant inspired by the anatomy of human hair. The procedure uses pre-loaded cartridges of 12 to 25 hairs and with a special harpoon anchoring system, drives them directly into the scalp. Performed under local anaesthetic, the procedure is minimally invasive and takes around 30 minutes in most cases. Improvements can be expected over the course of several weeks from this fast treatment requiring no downtime. Prices vary as it can be used on different parts of the body, including common problem areas, such as the hips and chin. TOP RESULTS As for breasts, Kiwi women are asking for more modestly sized implants than in previous years. "Of course, this varies between individuals, but by and large I use smaller implants in New Zealand than I do in Australia," says de Chalain. At present, patients have the choice of silicone or saline implants, but a new arrival on the market combines the benefits of both without the risk. The Ideal Implant is an FDA-approved, lifetime warranty, hybrid product offering the natural feel of silicone with the safety of saline inside. It's yet to reach our shores but given the waves it's making internationally, it will only be a matter of time.

An increasing number of luscious lips are the result of new tattooing technology. Lip tattooing isn't completely new, but the latest methods have taken the art to a new level. Rather than the harsh lip-liner look of the 90s, this treatment, adopted from Europe, promises a more natural result, great for those wanting definition, a soft flush of colour and the appearance of larger lips. What does the Nano Lip Contour treatment entail? Using different configurations of super-fine needles, we tattoo using an intelligent machine to implant colour into the top layers of skin for subtle definition. How is it different from traditional lip tattooing? Rather than artificial, block-like colours, we focus on creating nude and naturally flushed-looking lips, using colour in certain areas to enhance the shape of your lips. A little tweak in shape here and there can be a game-changer, although unlike traditional tattoos we don't go outside the vermillion border, which is the little edge around the lips. From there, we do all of the shading and shape-enhancing with a softer, wide-configuration needle, which spreads the colour lightly and ensures there are no seams or lines. There are several different patterns we use in this step to create different effects, such as a plumper bottom lip or a shapelier cupid's bow. All up, a session takes about two hours. What kind of ink do you use? We use Nouveau contour pigments in every single shade of pink coral and nude under the sun. We love this range for its diversity and longevity, and they fade really evenly, too. The pigments don't leave colour residue long term, and allow our clients to change their mind about the shade and the treatment. How many sessions does it take to complete? Two or three are usually required. The first session is generally about getting the shape and base laid down. The second session is mostly about perfection and blending. Sometimes a third is needed, especially when a client is seeking an undetectable result. What can we expect after the treatment? For the first five hours, clients usually experience some degree of swelling, but it returns to normal quickly. For five days after treatment, you'll need to apply a healing cream that reduces colour loss and stops the tattoo flaking off. Your lips will feel tight for a few days, and you may experience dryness. How long do results last? Id expect 10-14 months. I recommend a yearly top-up. Are there any skin tones you can't treat? The only effect we can't create is one lighter than a client's natural lips. It's not a service we provide, as white iron oxide, which is present in pastel shades, stays in the skin forever. Does it hurt? It doesn't. At the very worst, it's a tiny bit scratchy towards the end. We always err on the side of caution and nothing that we create in the first session can't be changed.
