Purpose of ICE Alert Provide An Information retrieval system for medical first responders to help protect lives ANYWHERE (at home, in public, overseas) and to facilitate greater patient mobility and peace of mind for carers Long range plan - Mission statement 1) Fulfil an urgent need to provide a universal patient information retrieval system for use by vulnerable adults ANYWHERE, that no other service club or commercial organisation has fulfilled in a single unified system ICE Pack; 2) Target future developments (Phase 3) towards all vulnerable patient groups including the hard to reach groups; 3) Bring together all Stakeholders and UK service clubs to explore together future improvements, in a shared Phase 3 project for targeting a re-launch in 2015-16 2

Target - Phase 2 Vulnerable Patient groups Help vulnerable adults who are frail or with prediagnosed medical conditions ANYWHERE at home, in public or abroad on vacation Typical suffers of debilitating medical conditions Diabetes Heart & circulation Asthma Epilepsy Kidney Failure Anaphylaxis reactions Alzheimer s Stoke Organ transplant or with implants Anti-coagulants Taking strong medication Other relevant emergency conditions... (if in doubt consult your GP or Pharmacist) 3

Firstly, read this Instruction leaflet carefully before starting to use the ICE Pack. I x ICE User instruction leaflet (A6) 2 x ICE Sheets (A6) Check ICE Pack 1 x ICE band (adult wrist band) contents and any sign 3 x ICE Surface wallets (A7 - S/Adhesive back) of damage. Replace if 1 x ICE Pocket wallet (A7 - clear re-sealable) needed. 2 x ICE Warning labels (50x75mm) 1 x ICE Warning label -Look in (50x75mm) 4

NEED HELP Contact a FAMILY member, a FRIENDS, a NEIGHBOUR or CARER or typically one of the following organisations IMPORTANT Protect yourself and your information at all times by; 8

Complete every section relevant ONLY to you, use a separate sheet/wallet for anyone else accompanying you or in your place of residence. IMPORTANT to update with changes immediately UPDATE - immediately if your information changes particularly MEDICATION 9

Fold the finished A6 ICE sheet in half, along the red dotted line to A7 size, ensuring to make the fold flat by using a blunt tool (i.e. the back of a spoon). OR A7 Wallet (74x105 mm) Place the ICE Sheet into the open end of the ICE Surface Wallet and slide it inside, with either of the two faces (depending on usage) upwards through the clear plastic 11

Sealing the ICE Sheet into the Surface Wallet (with a self adhesive backing) Remove the smallest strip of self adhesive backing paper attached to the opening flap. Fold over the sticky flap until it seals on the wallet (free of release paper). Peel the whole of the backing paper off, being careful not to accidently stick it to anything particularly your fingers or clothing. 12

Guidance on applications for Surface mounted Wallets OPTION A Entrance door Carefully position and attach the sticky side to the inside entrance door on the vertical frame at eye level in clear view. NO OTHER labelling required as the Medical first responder will see it and rip it off on his way into the property, to gain access to the ICE Sheet information. Property entrance door 13

Guidance on applications for Surface mounted Wallets OPTION B Refrigerator Place the vertical structure Wallet at the back of the refrigerator door above the bottles of milk, in full view to recover by ripping it off. Secondary Signpost In the Kitchen to show which door the refrigerator is behind Primary Signpost Place a warning label on the inside of the property entrance door and if necessary on a second entrance at the rear if widely used 14

Guidance on applications for self adhesive backed Surface mounted Wallets OPTION D Outside of the HOME (e.g. Vehicle storage and Travel Case) NO OTHER labelling required as the Medical first responder will see it and rip it off. Primary Signpost ICE Warning Label attached to the outside of the suitcase or label tag Position and attach the sticky side to the inside of the Suit case cover 16

ICE Alert use in Public SIGNPOSTING can be innovative (must be in clear view for first responders) ICE Bands can also be attached to other equipment used by service users ICE Warning labels attached to mobility aids Walking sticks Walking frames (Rolators) Mobility scooters 19

Guidance on storage and use of ICE Sheets in grip sealed Pocket wallets Wear the ICE Band on the wrist at all times in public. It can be worn at home as well, some service users leave it on even whilst sleeping, showering and bathing. If any swelling or irritation persists seek medical help. Carry an ICE Sheet at all times in public. Place the ICE sheet folded in half from A6 to A7, into the ICE Pocket wallet and seal it. It is optional to write your name on the wallet, particularly if a partner is travelling with you and also has an ICE Sheet. The Pocket wallets can be stored in many places typically a coat hand bag, a glove compartment of a vehicle, or suitcase 20

Guidance on use of ICE Alert in conjunction with other ICE products Service users already using another ICE product e.g. Lions message MiaB can still use elements of the ICE Alert Packs, for use in public (see the example a secure sheltered independent living residence below) Separate apartments on entering the secure building Attach the ICE Alert or MiaB Warning label to the entrance door Options Fridge or Wall Unit 21
