OVERVIEW The Boston Public Health Commission (BPHC) protects the health protects the health of Bostonians and visitors to the city through monitoring and prevention activities, inspections and emergency response to hazards, outreach and education, and enforcement of regulations. BPHC is seeking the services of a professional, experienced, certified vendor to provide bio-hazardous clean up, and removal and decontamination. Services to be provided at BPHC properties which include but are not limited to office buildings, client housing, client homes, and vehicles. SCOPE OF SERVICE The vendor shall furnish all necessary materials, equipment, labor, and supervision, equipment to provide biohazard waste clean-up, removal and disposal on a "per request basis in accordance with this RFQ and all appropriate Federal, Commonwealth of Massachusetts and, local laws and regulations. All vendor s personal safety equipment for staff must be provided by the vendor. Any services provided is to include preparation, management and labor for cleanup of biohazard. Vendor will dispose, process, and/or destroy materials only at a facility authorized to process such materials by the Commonwealth of Massachusetts and within the United States. Waste shall not be processed and/or disposed of in States that do not regulate the processing of biohazard waste materials. Vendor is solely responsible for any and all spills or leaks occurring during of any requested service and agrees to clean any such spills, leaks, or contamination. Vendor may perform services in which human and/or human bodily fluids are either known or suspected to be infected with National Institute of Health (NIH) risk group 4 and 3 agents (See Appendix). Additionally, the vendor may perform cleanup service of contaminated water. The vendor will be expected to perform the following services for locations, objects, furniture, and/or structural materials that are contaminated with biohazard waste (see Appendix for definition): Clean-up Disinfect Deodorize Sanitize Disposal Locations of service and scenes to include but not limited to: Trauma scene Crime Scenes Client Housing Homeless shelters Vehicles Businesses Page 2 of 5
QUALIFICATIONS Qualified vendor must fulfill the following requirements: Must have a minimum of five (5) years experience in bio-hazard cleaning Must provide immediate response service twenty-four (24) hours a day, seven (7) days a week, fifty-two (52) weeks a year Must have biohazard placarding on its vehicles Must provide confidential service Must be Occupational Safety and Health Administration (OSHA) compliant, and have ongoing medical waste handling, transportation, and process training for all vendor s all employees. Must have certified blood borne pathogen technicians All chemicals used for any service must be approved by the United States Environmental Protection Agency (EPA). Must maintain ongoing training for their employees and provide evidence thereof. At a minimum, the training must include the Bloodborne Pathogens Standard (29 CFR 1910.1030), the Hazard Communication Standard (29 CFR 1910.1200), and job specific Standard Operating Procedures (SOPs) Must train their employees on cleanup SOPs specifically designed for Ebola cleanup of various locations and provide employees with instructions on what to do if potentially exposed to Ebola. Awarded vendor will be required to provide documents proving destruction of any bio-hazardous waste generated Must maintain and provide a copy of general liability insurance, environmental liability insurance, and workers compensation insurance, and medical waste transporter license Provide statewide service across and within the Commonwealth of Massachusetts Will provide the name and registration number of medical waste disposal facility used Awarded vendor will provide CORI Affidavit PERIOD OF PERFORMANCE Services are expected to be performed by the company on or about January 1, 2018 through January 1, 2019. Appendix: Biohazard Waste: Human Bodily Fluids: may include but is not limited to blood, vomit, urine, feces, saliva, semen, vaginal secretions, amniotic fluid, and pleural fluid. Sharps Waste: Used needles or any sharp object (scalpels, glass slides, broken glass) which are contaminated with potentially infectious materials Pathological Waste: any human or animal tissues or parts Animal Waste: excreted materials from live animals Microbiological Waste: Discarded specimen cultures, disposable culture dishes Risk Group 3 (RG3): Agents that are associated with serious or lethal human disease for which preventive or therapeutic interventions may be available.
