Item #2 Current Price List, is hereby attached and made a part of this addendum. Item #3 Section 2.1 Calendar of Events is hereby revised as follows: September 11, 2015 September 15, 2015 September 18, 2015 October 2, 2015 Proposals due. Public bid opening at 2:30 PM Eastern Time at the location specified in Section 2.11. Proposal Guarantee due. Anticipated Date of Posting of Intent to Award. Anticipated Date Bid Awarded. Performance Bond due within ten (10) calendar days of award. Anticipated Date that Awarded Vendor shall provide sample(s) as outlined in Section 3.6, Pre-Production Samples. All date references reflected throughout ITB 003-16 are revised to reflect the above changes. ITEM#1 DHSMV ITB 003-16 FHP UNIFORMS AND UNIFORM ACCESSORIES QUESTIONS AND ANSWERS Questions received from GALLS: Question #1: Please provide your current contract pricing. Answer to Question #1: See Addendum No. 1, Item #2. Question #2: Will FHP consider staying in a 75/25 poly wool blend and not transition to the poly/wool/lycra if the samples are deemed an acceptable alternative? If not, can a separate price be submitted for both the poly/wool and the poly/wool/lycra blend? Answer to Question #2: No; FHP will not consider staying in a 75/25 poly wool blend. Separate pricing will not be accepted since continued use of the 75/25 blend is only a temporary 6 month transition period. Question #3: With the complexity of this bid will the FHP consider adding a two week extension for both the bid submittal and the pre-production samples required? The short turnaround will be challenging for any manufacture not currently servicing your account to produce the adequate samples. Answer to Question #3: Yes, See Addendum No. 1, Item #3, and Item #4. Question #4 Will a ramp up time be permitted if FHP awards to a new vendor / manufacture? Answer to Question #4 The ramp up time period will be from sample acceptance until January 1, 2016. Thereafter, the awarded vendor will need to maintain sufficient stock to meet all fill requirements. Answer to Question #5 Yes; the award will be made to the responsive and responsible vendor meeting all specifications, terms, and conditions of this solicitation with the lowest total bid price (A-HL) = (HM) for both 5 year periods. Question #6 Will there be escalation of pricing allowed in subsequent years of the contracts? Answer to Question #6 See Attachment I, PUR 1000 subsection 4(e), Pricing Escalation, on page 164 of ITB 003-16. Question #7 4.1.1 Female Fla Hwy Patrol Trooper Trouser and 4.1.2 Male Highway Patrol Trooper Trouser upon change into new fabric will mixed inventory be allowed? Answer to Question #7 Yes. The inventory can be mixed during the first 6 months of the contract only. Implementation of the Lycra into the blend must be complete on orders filled 6 months after the contract start date.
