1 EduCare Demo Skill Assessment Personal Cares Employee Name Demonstration General Procedure for All Personal Cares 1. Read the Care Plan. 2. Knock before entering; identify yourself. 3. Greet client by name. 4. Explain what you will be doing. 5. Protect dignity; offer privacy. 6. Wash hands; wear disposable gloves. 7. When procedure is completed - dispose of gloves; wash hands. 8. Document the care that you have provided. Date Dressing & Undressing 2. Encourage client in the choice of clothing, undergarments, shoes, and accessories. 3. If a client has a weak side, always begin dressing from that side. 4. Remember that safety is important. Always be certain that the client has their balance or is sitting down when possible. 5. After undressing, always put soiled laundry in the client s laundry basket. Bathing 2. If necessary, assist client in undressing. 3. Turn water on. 4. Adjust water temperature. 5. Test water temperature and allow the client to test it also. 6. Wash your hands and apply gloves. 7. Help the client into the shower or bathtub slowly. 8. Help client sit on shower/bath chair. 9. Offer client a washcloth & soap. 10. Encourage independence. 11. Wash the client s entire body starting with the face with soap and water. End with the peri-area. 12. Assist with rinsing all areas. 13. Towel dry body. 14. Observe changes in skin condition. 15. Assist the client out of the shower or bathtub. 16. Help to apply toiletries. 17. 2 Sponge Bathing 2. Remove clothing and cover with bath blanket or towels. 3. Wash your hands and apply gloves. 4. Offer washcloth to client to do as much for herself as possible. 5. Wash one area at a time starting with upper body and working toward the feet. Wash the periarea last. 6. Dry each area as you go making sure that areas of the body are not exposed. 7. As each area is washed and dried, cover with clean towel to keep warm. 8. Apply lotion if desired. 9. Remove gloves and wash your hands. 10. Assist with dressing. Oral Care (Teeth and Gums) 2. Bring the client to the sink. 3. Wrap towel around neck and chest to protect clothing. 4. Wash your hands and apply gloves. 5. Encourage client to moisten mouth with water. 6. Encourage client to place tooth paste on the toothbrush. 7. Prompt or cue client to begin brushing teeth. 8. If necessary, demonstrate how to brush teeth. 9. If necessary, assist client with brushing. 10. Reminder client to brush well and rinse mouth with fresh water. 11. Offer a towel to dry mouth and hands. 12. Remove gloves and wash your hands. 13. Clean supplies and sink area. 14. Return supplies to proper place. 15. Check condition of toothbrush; if worn out, replace. Denture Care 1. Wash your hands and apply gloves. 2. Put a towel under the client s chin to protect clothing. 3. Have client rinse their mouth with water to loosen dentures. 4. To protect dentures from breaking if accidentally dropped, place washcloth or towel in the sink. 5. Encourage client to remove dentures from mouth or assist if necessary. 6. Place dentures into denture cup. 7. Soak dentures in a denture cup with cleaning product or use a toothbrush and toothpaste to clean. 8. Assist client to rinse their mouth with mouthwash. 9. Wipe the client s mouth with a towel. 10. Remove gloves and wash your hands. Mirabelle Management, 2007 Page 2 of 5

3 Perineal Care: Female 1. Gather or use supplies assembled. 2. Wash hands; put on gloves. 3. Gently separate the female labia, wash, rinse and dry the urethral area first with downward strokes alternating from side to side. 4. Rinse the cloth. 5. Wash the groin outside of the labia from front to back and then inside of the thighs. 6. Rinse the cloth. 7. Wash, rinse, and dry the rectal area. Perineal Care: Male 1. Gather or use supplies assembled. 2. Wash hands; put on gloves. 3. Wash groin from front to back and then inside the thigh area. 4. Rinse the washcloth. 5. If uncircumcised, pull back foreskin and wash the penis downward using circular motions and then wash the scrotum. 6. Rinse the cloth. 7. Wash, rinse, and dry the rectal area. Toileting 1. Wash your hands and apply gloves. 2. Assist the client onto the toilet. 3. Allow the client to void. 4. Prompt or assist with cleaning the peri-area. 6. Replace undergarments or incontinent product. 7. Assist the client off of the toilet. 8. Prompt or assist client to pull garments up. 9. Remove gloves and wash your hands. 10. 4 Shaving 1. Ask your client how he likes to shave. 2. Place a towel around his shoulders. 3. Wash your hands. No gloves are needed. 4. Soften the whiskers with warm water and a wash cloth or use a shaving product designed to cause the whiskers to stand up. 5. Use gentle pressure against the beard. 6. Shave against the grain of the beard growth. 7. Do not press down hard or move the shaver too rapidly over the face. 8. Shave sensitive areas first, such the neck area below the jaw bone. 9. Pay special attention to any little folds in the skin, especially to the creases at the corner of the mouth and around the nose. 10. After shaving, wipe the face with a moist washcloth and towel dry. 11. Offer to apply aftershave or moisturizer. 12. Clean whisker dust out of the cutter blades with the shaver brush. 13. Return razor to the charger. 14. Wash your hands. Foot Care Foot Care While Bathing 1. While wearing the gloves you applied on your clean hands for the bath, wash feet in between the toes with warm soapy water. 2. Rinse and dry thoroughly. 3. Do not apply lotion between the toes. This area needs to be kept dry. 4. You may apply lotion to the rest of the foot, especially the heels. 5. Encourage wearing moisture-wicking socks. Foot Care Without Bathing 1. Arrange supplies and equipment. 2. Fill basin with warm water & soap. 3. Test water temperature. 4. Wash hands; put on gloves. 5. Remove client s shoes and socks. 6. Ask the client to place feet into the basin and soak feet for several minutes. 7. Follow the same procedure for foot care as previously described. Fingernail Care 2. Wash the client s hands or create a basin with warm water and soap and soak hands in basin. 3. Dry hands thoroughly. 4. Smooth rough edges with emery board or file. 5. Wash your hands. 5 Installing Hearing Aid 2. Ensure that the hearing aid is turned on. 3. Best to have client place in ear with your guidance. 4. Ensure that the right hearing aid is in the right ear and the left is in the left ear. 5. Ensure that the client can hear. 6. Adjust volume if there is a whistling sound. Changing Hearing Aid Batteries 2. Open the door/chamber. 3. Remove the old battery. 4. Position the new battery correctly. 5. Place in the slot. 6. Close the door/chamber. 7. Adjust volume if there is a whistling sound. Cleaning Hearing Aids 1. Wipe the hearing aid with a clean cloth or facial tissue. 2. Inspect and remove ear wax accumulation. 3. Use manufacturer s brush/wire pick to clean. 4. Carefully brush around and in the plastic tubing to clear out the wax, being careful not to force the wax in deeper. 5. You can use a designated toothbrush just for cleaning the hearing aid. 6. Open the battery door to allow air to enter less moisture and extends batteries. 7. Place the hearing aid in its storage case or a designated small dish or container. 8. Ask supervisor for guidance if unfamiliar with a client s brand of hearing aid as these are expensive devices. Stand-by Assist 1. Wash your hands, gloves are not necessary. 2. Gather the necessary supplies. 3. Hand the supplies to the client in the order used. 4. Ensure the client's safety. Do not become distracted. 5. Offer assistance, only when needed Shampooing 1. Wash your hands, gloves are not necessary. 2. In shower is the easiest method or kitchen sink with sprayer 3. Have the client can sit on a shower chair 4. Massage the shampoo into the scalp 5. Rinse and repeat 6. Rinse thoroughly 7. Towel dry and style per client s request.
