3 Waiver Renewal & Policy Changes Appendix B: Modified the process for annual recertification of level of care criteria. Effective July 1, 2017, the reevaluation process will utilize SIS standard scores to determine if the waiver participant continues to meet ICF/ID/ORC Level of Care If you have questions regarding this new process, please reach out to your regional ODP LOC Lead 9/1/2017 3

5 Waiver Renewal & Policy Changes Continued Policy: Age range - ODP, with permission from AAIDD, changed the age range from as young as 14 and older than 72 years of age. Everyone will receive a SIS Roll-out phase Priority groups Individuals who receive residential services will require the presence of at least one residential staff as a qualified respondent in order to complete the assessment. 9/1/2017 5
