2 JUNIOR LEAGUE OF THE LEHIGH VALLEY Women building better communities MODEL AND LIABILITY RELEASE For good and valuable considerations, receipt of which is hereby acknowledged, and intending to be legally bound thereby, I, on behalf of myself and my minor child, hereby consent to and grant to The Junior League of the Lehigh Valley, 3477 Corporate Parkway, Suite 100, Center Valley, Pennsylvania (the "JLLV"), its legal representatives and/or assigns, or anyone authorized by the JLLV, the absolute right and privilege to use, re-use, publish and re-publish and copyright worldwide and in perpetuity for any and all purposes, in any and all forms of media, without further limitation, restriction, compensation, notice, review, or approval, any and all film, photographs or pictures of my minor child or in which my minor child is included, in whole or in part, in composite or distorted in character or form, without restriction as to changes or alterations, and of any reproductions thereof in color or otherwise, which film, photographs, pictures, portraits, images or likenesses will be taken by the JLLV, or its agents and assigns, at the Spring 2016 American Girl Fashion Show at Lehigh University, Bethlehem, Pennsylvania (the Event ) or any adjunct to the Event. I also consent to the use of my minor child s full name, city and state of residence, photographs or other likenesses, pictures, portraits, voice, testimonials, biographical information, statements or interviews (in whole or in part) in connection with or without the said photographs or the Event. All of the aforesaid rights granted herein shall extend worldwide and in perpetuity to any lawful purpose whatsoever, including, but not limited to, the sale, publication, display and exhibition thereof in editorial, promotion, advertising, trade, promotion or otherwise on behalf of the JLLV and in any and all media now known or hereafter devised, including, but not limited to, print, TV, radio, cable, electronic, or World Wide Web, without further limitation, restriction, compensation, notice, review or approval. I, on behalf of myself and my minor child, agree the photographs and negatives thereof, photographs or other likenesses, pictures, portraits, voice, testimonials, biographical information, statements or interviews shall constitute the JLLV's sole property, with full right of disposition in any manner whatsoever. I, on behalf of myself and my minor child, acknowledge I must supervise my minor child at all times during the Event and any model call for the Event, and that supervision of my minor child is my sole and exclusive responsibility. Furthermore, I, on behalf of myself and my minor child, hereby remise, release and forever discharge the JLLV and its governing board of directors and boards, committees, officers, agents, employees, servants, ostensible agents, members, licensees, and assigns, from and against any and all liability, claims, demands, actions, causes of actions, costs and expenses of any nature that may arise out of or relate to the Event and model call for the Event, including any loss, damage or injury that may be sustained by me or my minor child or any personal property belonging to me or my minor child, while at, participating in, on,

3 upon, or in transit to or from the Event and model call for the Event or any adjunct to the Event or model call for the Event. I hereby warrant that I am the parent or legal guardian of the minor child identified below, and that I have the right to contract on behalf of my minor child. This release shall be binding upon me and my minor child, and our family, heirs, legal representatives, and assigns. I have read the above authorization, release and agreement, prior to its execution, and I am fully familiar with the contents thereof. 4 The Junior League of the Lehigh Valley is looking for models for our 2016 American Girl Fashion Show! The Junior Leage of the Lehigh Valley s Model Search will be held on the following dates: October 3rd at Sophistikate in Bethlehem from 10 am - 2 pm October 17th at Dreyus Kids in Easton from 10 am - 2 pm October 24th at Sweet Pea Children's Boutique in Bethlehem from 10 am - 2 pm Clothing is provided in size 10 and 6X: Size 10-Height tall. Size 10 girls will model American Girl historical and contemporary clothing. Size 6X-Height 46 to 48 tall. Size 6X girls will model American Girl Little Girl Clothing line. All models will be required to attend 1 rehearsal prior to the show. More information regarding fittings and rehearsal times will be announced at a later date. Models may be required to supply footwear and socks/tights depending on the costume. We are looking for girls with poise, confidence and a smile who would enjoy walking down a runway in front of an audience of 240 guests. 5 Important Information 2016 American Girl Fashion Show Presented by Junior League of the Lehigh Valley Saturday, March 5, AM & 2 PM Sunday, March 6, AM & 2 PM MODEL CRITERIA & REQUIREMENTS: Models must be age five or older. The fashions worn during our shows will be sent to us by the American Girl Company and are sizes 6x and 10. All models will be required to attend a fitting on Saturday, February 13 and a rehearsal in February. Information regarding rehearsal times will be provided at a later date. We will have four shows. Ticket prices for all shows will be approximately $35-$47 and will include refreshments. HOW ARE MODELS SELECTED? Every girl must attend an audition to be considered. American Girl provides us with all of the clothing for the show and specific measurements for each outfit. Your daughter will be selected based on measurement primarily. Girls may not select the outfit they model. We will select the girls who best fit the outfits. PLEASE let your daughter know that this is NOT A PAGEANT we are not selecting on talent or looks ONLY SIZE & in some cases, likeness to the dolls according to each outfit we have in the show. We don t want anyone to be disappointed if they are not selected. (See If your daughter is not selected below) Please keep in mind that this event is a fundraiser for the Junior League of the Lehigh Valley (JLLV). With your help, the JLLV will help the Lehigh Valley community through its children s projects. Good Luck and Have FUN! Size 6x will model American Girl Bitty Baby clothing (not American Girl Doll clothing). WHEN WILL MODELS BE SELECTED? Models will be selected after the audition. We do our best to select the girls and notify parents within two weeks. IF YOUR DAUGHTER IS SELECTED: She is required to attend a fitting/photo shoot on February 13, 2016 at Iacocca Hall at Lehigh University. At the fitting, the model will try on her outfit and be photographed with the matching doll. If she cannot attend the fitting, please contact us for alternate arrangements. She is required to attend a rehearsal in February at the Bethlehem Township Community Center. Rehearsal date will be based on show assignment and a schedule will be available at a later date. She will not need to purchase a ticket to the show because she will be backstage. All parents, siblings and guests wanting to watch the show will need to purchase tickets in advance. Some shows sell out very quickly! The Fashion Show Modeling Package - $40 includes: Modeling spot in the 2016 production of the American Girl Fashion Show. Models will borrow our clothing and dolls for the photo shoot and again at the show. The clothing and dolls remain the property of American Girl. Professional photo shoot with Mandy Suro Photography. 5 x7 print from photo shoot. A special t-shirt made exclusively for our models. An official American Girl Fashion Show gift bag Corporate Pkwy, Ste. 100 Center Valley, PA agfs@jllvpa.org IF YOUR DAUGHTER IS NOT SELECTED The $40 modeling fee will be returned to you, but she can still walk the runway at intermission as a V.I.P. Guest. Limited spaces are available. Package ($25) includes: Opportunity to walk the runway at intermission as our MC shares information about our V.I.P. Guest with the audience. Models will walk with their own dolls! Exclusive American Girl Fashion Show t-shirt for model to take home. Model can wear her own current American Girl clothing if applicable.
