The warning comes after the publication of the College of Optometrists latest Optical Workforce Survey, which found that there are currently 12,099 full-time equivalent optometrists in the UK, whereas 12,912 might be needed to meet the current needs of the population. As a result, a net undersupply is predicted. This, and several others identified trends, are to be the focus of a sector-wide response and investigation into the likely causal factors, such as a possible fall in remuneration, the potential disparity in women s pay and the apparently low numbers of clinical appraisals being carried out. The College is planning a roundtable discussion in the spring to discuss the issues, and we will be keeping members updated. As is customary, our March issue focuses on what NEG s preferred suppliers will be showing and promoting at Optrafair, this year being held from 7-9 April at the NEC Birmingham (see pages 19-25). 4 2 NEWS NEWS Daniel 1 Optical Confederation Alistair Burt MP addresses an optical audience in London Health minister, Alistair Burt, told delegates at 100% Optical last month that patients shouldn t see GPs for every medical ailment, and that clinical commissioning groups (CCGs) need to use the whole range of health professionals, including those in eyecare, to innovate in health delivery. Speaking for the first time to an exclusively optical audience on the Saturday of the London show, the minister responsible for primary care services said that he was determined to ensure that the development of primary care was not centred solely around general practice and that all primary care professions were recognised and valued. The role of primary care in developing services for the future is crucial and it s vitally important that this includes eyecare professionals, shaping eyecare services so they are fit to meet the growing demand for the future, the minister said. He added that it was important that CCGs worked in partnership with the wider workforce in primary care when deciding what was best for patients. Radical change is required to realise our objectives for health and wellbeing, quality and efficiency and to simply be able to meet the challenges we are going to face with a growing population, and to meet the increasing needs of those with visual impairments. However, doing this is not just something for the CCG in isolation, but something in which the local health community has a shared interest and should involve a full range of healthcare professionals. Read more at 2 Silhouette This summer will see the launch of a new Made in Austria eyewear brand from Silhouette Neubau Eyewear. The inspiration for the collection comes from the hub of urban creativity that inhabits Vienna s 7th District. Presented for the first time at Opti Munich in January, the collection for men and women was also previewed at 100% Optical in London last month. It includes information about how to apply the new Standards of Practice competency, which come into force on 1 April. The booklet also provides guidance on how to make greater use of the Personal Development Plan (PDP), which allows registrants to reflect on their scope of practice, identify learning goals and further their professional development. GOC acting head of education and standards, Marcus Dye, said: We have taken on board feedback from registrants in making clearer some aspects of MyCET, and have included further guidance on how registrants can apply to provide peer review sessions through the system. I d particularly like to encourage registrants to make use of the personal development plans to plan their professional development and complete reflection statements as a way of considering how they can apply their CET in practice. 7 NEWS 5 6 College of Optometrists 8 No7 Contact Lenses A new Optical Workforce Survey published by the College of Optometrists has identified a future under-supply of optometrists in the UK, and highlighted the continuation of a gender pay gap. Designed to measure changes in the optical workforce, with data collected from dispensing and contact lens opticians for the first time, the study was led by the College with support from a cross-sector advisory group. It found that 80 per cent of optometrists and 79 per cent of DOs questioned were satisfied with their job, but estimated that there were currently 12,099 full-time equivalent (FTE) optometrists in the UK, whereas 12,912 might be needed to meet the current needs of the population. As a result, there might be a net undersupply. Mike Bowen, director of research for the College of Optometrists, said: Workforce planning relates not just to the number of new recruits to the sector, but also how to develop new skills and working patterns for those who are already employed. Although this study did not directly aim to determine if there is an over or undersupply of optical professionals in the UK, the results, supplemented by additional data sources, can be used to identify trends that relate to current and future workforce patterns relative to demographic changes and changes to service provision. The data also raises other questions that require further investigation and a sector-wide response, including the factors contributing to oversupply and undersupply in certain regions, a possible fall in remuneration, the potential disparity in women s pay and the apparently low numbers of clinical appraisals being carried out. These issues, and a range of others highlighted in the report will be the subject of a roundtable event involving representatives from across the sector. The event will be hosted by the College of Optometrists in spring 2016, Mike added. Read the report at 7 CooperVision A new white paper commissioned by CooperVision contends that eyecare practitioners (ECPs) who focus on innovation are more likely to build customer loyalty and, in turn, their practice. Written by an independent author, the paper investigates the relationship between innovation, customer loyalty and business growth within the optometry industry, drawing comparisons from other relevant sectors. Taking insights from more than 70 sources, the paper is interspersed with quotes from personal interviews with ECPs representing national brands, small chains and independents. It also includes advice on how to put the theories into practice. Focus on innovation encouraged The paper suggests that loyal customers provide a reliable income and save the ECP time in trying to develop new business or delivering a hard sell. 9 NEWS 7 11 Association of British Dispensing Opticians 13 General Optical Council The General Optical Council (GOC) has agreed a new policy providing guidance for registrants and others in the optical sector on raising concerns about risks to patient safety, including where to go for advice and what protection is offered. The policy has undergone a period of consultation with stakeholders and the GOC will publish the final version shortly. It sets out the steps that registrants or others working in the sector should take if they believe that patient safety or care is being compromised by colleagues or the organisations in which they work or study. This might be, for example, because they are concerned about a colleague whose fitness to practise may be impaired, that record keeping or sight test processes are insufficient, fraud or false advertising, or that patient safety risks are being covered up and not addressed. Read more at 14 Safilo Incoming ABDO president, Fiona Anderson, being filmed at Optrafair 2015 The Association of British Dispensing Opticians (ABDO) and ITN Productions have announced that for the third year running, they will be entering into a partnership to produce a news and current affairs-style programme, which will explore the latest innovations and achievements in the profession of dispensing opticians. Introduced by national newsreader, Natasha Kaplinsky, the programme will feature key industry interviews and news-style reports along with sponsored editorial profiles of some of the leading organisations. The programme will aim to provide a greater understanding of the role of opticians examining the most important challenges and opportunities ahead, as well as capturing the very latest developments and best practice, particularly in the area of paediatric dispensing, lens technology and spectacle frames. Simon Shelley, head of industry news, ITN Productions, said: ITN Productions has enjoyed its partnership with the Association of British Dispensing Opticians, and we re keen to embark on another programme exploring the nation s eye health. By bringing new innovations and solutions to screen, and looking into the organisations and the passionate people behind them, we hope to spark new dialogue that could shape the future, he added. Open 30 Presbyo is available with an 8.70mm base curve, 14.10mm diameter and powers ranging from +6.00D to -6.00D. 16 Association of Optometrists Ireland Eyewear deal made Optometrists in Ireland have called on Election Groupings and Candidates to support community based services to improve eye health, in a General Election 2016 Manifesto. The Association of Optometrists Ireland (AOI) said that visual impairment and blindness were significantly on the increase in Ireland, with an estimated 75 per cent of eye disease preventable through regular eye examinations. AOI chief executive, Peter Coleman, said: Increased eyecare services in the community would reduce waiting times for hospital out-patient and treatment services. By providing more timely treatment, the level of vision impairment and blindness can be reduced. Investment in cost-effective community based eyecare interventions can save the state an estimated 76m in treatment costs.

The colour style PU offers a striking chameleon colour effect. 21 AIO The Association for Independent Optometrists and Dispensing Opticians (AIO) has welcomed the move by the General Optical Council (GOC) to revise its standards in respect of business registrants alongside the revision for individuals on 1 April The Association had strongly encouraged the GOC to take this approach during its consultation on standards, to ensure that there was consistency in how the profession operated across the board and in what the public should expect on their eyecare provider. There is a clear difference in operating models between smaller independent practices and the larger corporate operators, and it is important that standards are consistent across different business models, particularly where a clear employer/employee model for professionals exists in larger companies, said AIO chairman, Peter Warren. We welcome the fact that standards for business registrants will be updated alongside and consistent with those for individual registrants. We are, however, disappointed that Standards for Students have been introduced, particularly when they have not been limited to patient interaction. It is not clear why the world of optics should be unique amongst other medical professions in requiring this. 22 Mondottica Mondottica has announced the signing of two new eyewear licenses for Cath Kidston and Le Coq Sportif. The license with the iconic Cath Kidston brand is worldwide and is targeted to major retailers initially in Europe and Asia. It will include optical frames, sunglasses and readers. It will also extend to designer prescription contact lenses for the Asian market. The licence with Le Coq Sportif is also targeted to key retailers initially in France and Switzerland. Group commercial director, Steve Tulba, said: We are delighted to partner with Cath Kidston and Le Coq Sportif to create the first eyewear collections for both brands. They both offer something very unique, adding even more diversity and global appeal to the current stable of Mondottica licensed brands.

The study also found that of those who screened positive for depressive symptoms, almost three-quarters (74.8 per cent) were not receiving any treatment for their depression. Currently, just two low vision services in Britain regularly screen patients for depression. The three-year research trial, commissioned by the charity Guide Dogs and led by a team of researchers from Cardiff University, found that diagnosis and treatment for depression for people attending low vision clinics was routinely overlooked. Held at ExCel London from 6-8 February, highlights included Department of Health minister, Alistair Burt, discussing the role of optometrists and dispensing opticians in delivering a sustainable NHS and Professor Kovin Naidoo covering the increase of myopia in his talk, Myopia: A growing clinical and public health challenge of our time. Association of Optometrists (AOP) chief executive, Henrietta Alderman, said: The AOP are delighted with our ongoing partnership with Media 10, and this year delivered a show that lived up to and beyond expectations. The show delivered world class education provided by the AOP, a fantastic exhibition and an opportunity to network and bring the whole sector together under one roof, continued Henrietta. This year s programme also provided plenty of opportunity for topical debate within the 90 plus sessions of CET, including hearing from the minister who is responsible for optics, Alistair Burt MP, who gave delegates his view of the challenges we face and the positive role he feels we should play within primary care. In a statement, Essilor International said the move further strengthened its current positions in online optical retail in Europe, by complementing existing activities in the Nordic countries. Vision Direct s existing management team will remain in place. AN INDEPENDENT VIEW New programme for aspiring independents The AIO has long had the intent to provide help to optometrists and dispensing opticians who aspire to enter into independent practice. The Association has been in dialogue with a variety of different organisations as to how this help is best delivered, and in particular those suppliers to the optical profession that want to see a healthy and vibrant independent sector. The AIO council has now decided that it is time to take some positive action. In recent years, the AIO annual conference has grown into one of the go to events in the optical calendar. The event, which was formerly held for members only, is now open to all in the optical profession and delegate numbers have increased substantially as a result. As the conference has grown and the range of clinical and practical content expanded, the timing is right to introduce a special feature programme that is specifically tailored to inform and help aspiring independents. The new programme will include expert presentations on all aspects of setting up a new practice or acquiring an existing practice, and will feature presentations by professionals who have recently and successfully taken the step into independent practice. There will be specialist presenters talking about a range of subjects such as marketing, location, finance, use of technology and social media, as well as the practical aspects of acquiring state-of-the-art optical equipment. The prospect of becoming a business owner can be daunting and there is no doubt that this has been an inhibitor to growth in the independent sector in recent years. With the huge changes in the provision of optical services on the High Street, some may question the future viability of independent practice. However, there are literally thousands of independents who own successful and sustainable businesses, and there is clearly room for all optical service providers in the marketplace. It is a question of getting the basics of business right and letting the excellence of the clinical offering speak for itself. Whether newly qualified, working in an existing independent or indeed a multiple, the AIO conference in 2016 will offer a unique opportunity to help in the decision making process and making the move. 16 14 PRODUCT PROFILE MULTI-MODAL IMAGING: a safe investment The Spectralis multi-modal imaging platform continues to prove itself as a safe investment for eyecare practitioners (ECPs) Christopher Mody, director of clinical services at Heidelberg Engineering UK, discusses important considerations and new possibilities with two emerging OCT technologies: swept-source OCT and OCT angiography¹. Top: an image of the macular taken using Spectralis OCT; bottom: a follow-up image of the same eye taken in the same anatomic location using the Spectralis OCT2 and TruTrackActive Eye Tracking Next generation OCT Spectral domain OCT (SD-OCT) has become indispensable to ECPs for the diagnosis and follow-up of patients with diseases of the posterior segment, such as age-related macular degeneration (AMD), diabetic retinopathy and glaucoma. SD-OCT has been the gold standard in OCT imaging since its introduction in 2006, but new technologies have begun to emerge in recent years, such as next generation SD-OCT technology and Swept Source OCT. Swept-source OCT employs a fast scanning speed and longer wavelength than SD-OCT to allow for a deeper range of imaging of the retina and improved image acquisition time. Whilst these features may be desirable, it is important to consider the impact that using this long wavelength OCT has on image quality, clinical decision making and workflow. For example, using a long wavelength laser provides deep tissue penetration into the choroid; however, this is at the expense of resolution in the neurosensory (inner) retina 2. Whilst it has been suggested that the choroid thins in cases of neovascular AMD, and thickens in central serous retinopathy, there are many diseases in which the role of the choroid is not apparent or unknown. Therefore, trading off resolution in the inner retina for deeper choroidal penetration may not be clinically advantageous at this time. Furthermore, adopting a new imaging device in some cases renders historical patient data obsolete, removing the clinician s ability to monitor disease progression over time using images and data from their existing device. In short, it is important to balance the novelty of adopting a new imaging platform, with clinical relevance of the technology and the preservation of historical data. visualisation of the corneal structures and large scale architecture of the cornea, iris and crystalline lens. Heidelberg Engineering is at the forefront of swept-source OCT research 3 and will shortly introduce a new swept-source technology based platform to examine the anterior segment of the eye (Figure 1). This new platform integrates sophisticated imaging and measurement functions and will be the basis for a growing number of exciting analytical and imaging features in the future. SD-OCT technology has improved considerably since its debut with the introduction of faster scanning speeds, high resolution imaging and new imaging techniques emerging year on year, such as anterior segment imaging and OCT angiography. In 2015, Heidelberg Engineering introduced next generation SD-OCT technology for the Spectralis imaging platform, named OCT2, providing a high scan rate of 85,000Hz with Heidelberg Engineering s proprietary TruTrack Active Eye Tracking to provide excellent image quality and significantly improved acquisition speed (Figure 2). OCT2 allows the fast capture of high resolution OCT images from vitreous through to choroid. Crucially, OCT2 will be available as an upgrade to existing Spectralis devices, so patient data is preserved and it is possible to continue to scan existing patients in the same anatomic location, to within one micron, at every follow-up visit using the TruTrack Active Eye Tracking facility. OCT angiography: a new OCT application OCT documents differences in reflectivity within tissues such as the retina, whilst OCT angiography (OCT-A) analyses not only the intensity of the reflected Figure 1. 17 PRODUCT PROFILE 15 signal, but also the time changes in the reflection caused by moving particles for example, erythrocytes flowing through vessels. The result is a three dimensional (3D) reconstruction of the perfused microvasculature within the retina and choroid (Figure 3). Although the clinical evaluation of OCT angiography is ongoing, the clinical potential of this new technology is tremendous. Compared with traditional fluorescence angiography, this new method offers both clinical and practical advantages, although it does come with physical limitations. One very significant advantage of OCT-A is that it does not require a contrast agent, so the associated risks of dye injection are eliminated. For this reason, OCT-A can be performed more frequently than fluorescence angiography. In addition, OCT-A makes it possible to visualise distinct vascular networks at different depths in the retina, whereas fluorescence angiography does not offer this kind of spatial resolution. OCT angiography clearly reveals the extent of the neovascularisation The development of OCT-A is a clear indication of the rising demand for diagnostic innovation. Early clinical investigations have confirmed that the combination of OCT- A with other diagnostic imaging modalities will offer an improved understanding of many clinical conditions (Figures 4 and 5). Eye Tracking (Figure 6), which detects eye movements during acquisition and only saves datasets that were definitely captured without any eye movements. Moreover, the TruTrack Active Eye Tracking system repositions the OCT scan at the correct location on the retina if an eye movement has occurred. This Figure 6. Spectralis TruTrack Active Eye Tracking patented technology ensures that the dense 3D volume scans required for OCT-A can be taken without motion artefacts that would otherwise diminish the clinical value of OCT-A. Accurate segmentation of the retinal layers poses an additional technical challenge in OCT-A imaging (Figure 7). OCT-A makes it possible to visualise distinct vascular networks at different depths in the retina. Speaking figuratively, OCT-A allows clinicians to move through the vascular network of the retina layer by layer. Therefore, the challenge of OCT-A is accurate segmentation of the OCT image slab. In order to generate an OCT-A image, the software segmentation algorithm must be able to identify the anatomic boundaries where the arterioles, venules, superficial and deep capillary plexus vascular structures are located. Segmentation artefacts will impact upon the visualisation of the vascular networks. OCT-A offers great diagnostic potential for ophthalmology, particularly in terms of identifying and classifying degenerative changes in the perfusion behaviour of the retinal vasculature, and following these changes over time. Many experts agree that the procedure offers new, treatmentrelevant information in a wide range of applications and in some cases could even replace fluorescence angiography. Consider, for example, the numerous follow-up examinations involved in the course of intravitreal anti-vegf therapy. There is huge potential for OCT-A to track how degenerated vessels respond to treatment. From a patient s perspective, the possibility of an earlier, more comprehensive diagnosis and the opportunity to avoid many fluorescent dye injections are both significant advantages. However, at present, more intensive clinical investigations are required before specific recommendations can be made. Important considerations Figure 5. Images of proliferative diabetic retinopathy taken with infrared, fluorescein angiography and OCT-A imaging modalities Despite the enthusiasm, it is important to recognise that OCT-A technology does have some limitations. Static or very slow flow phenomena such as capillary leakage and polyps are not well visualised. In such cases, it is not possible to generate sufficient motion contrast. With OCT-A, it is also not possible to differentiate between arteries and veins in the same way as with the in-flow of dye in fluorescence angiography. In addition, interpreting the 3D images may pose a challenge as this is still a relatively new technique. Eye movements pose one of the greatest technical challenges in ophthalmic OCT imaging. In OCT-A, reflection must be measured repeatedly at exactly the same point on the retina. In addition, a large number of densely spaced cross-sections need to be acquired to achieve a high-resolution 3D dataset. Even with the fastest commercially available OCT components, OCT-A datasets can suffer from motion artefacts unless effective eye tracking is used. This poses a real problem and risk from a diagnostic perspective. To avoid this, Heidelberg Engineering uses a technology called TruTrack Active At this time, OCT-A is still in a clinical evaluation phase. To achieve reliable outcomes that can later be transferred to the use of OCT-A by private practitioners, experience needs to be gained under trial conditions. For this reason, OCT-A is currently mostly used in clinical settings. As with all fundus imaging modalities, OCTA will most certainly be proven to offer additional information for accurate referrals and treatment at the right time. For years, the Spectralis multi-modal imaging platform has allowed a more targeted approach for individualised treatment. This flexible platform can be expanded and updated with state-of-the-art technology offering patients long-term solutions. The new OCT2 module, which serves as the basic platform for OCT-A, is not only available for new devices, but will be available as an upgrade on a range of existing devices in the future. As the independent practice evolves in the UK marketplace, as do the suppliers and exhibitions. Optrafair is the largest and most established exhibition in the UK and the one in which the vast majority of companies decide to invest large sums of money to showcase their latest products. Optrafair is the perfect platform to progress the independent practice and keep your finger on the pulse with exactly what is happening on the High Street, and to find out what you can do personally to improve your business, clinical skills and patient experience. So how do I get the most out of an exhibition one may ask? The answer is simple: plan. Don t just turn up for a little browse and see what catches your eye. Plan, plan and plan. Before the show, do your homework. This starts with taking a look at what your practice actually needs and what you would like to achieve from visiting a show like Optrafair. Get to grips with your stock situation, equipment and suppliers and assess your requirements, formulating a shopping list of sorts. It's also the easiest area to get carried away with, so beware and structure your shopping list carefully and think ahead. Many frame companies plan their new releases around Optrafair and launch their new marketing material and point-of-sale at the show. You will be surprised how much your suppliers can help you with your in-house marketing and social media. Visit your trusted suppliers to take advantage of any promotions they may offer but don't forget to view the latest products from new companies. Again, it s a great opportunity to see product side by side without any commitment to purchase. Lenses and contact lenses Not to be forgotten, lenses are a vital and integral part of the practice portfolio. It is possibly the single sector with the most current advancements taking place with the introduction of CAD and digital design edging machines pushing the boundaries in terms of sports wraps, rimless and other specialist frame mounting. So it's ideal to check out the latest information sources and speak to local and nationwide Rx houses at the show. Planning your time is key and cost-effective, and always allow plenty of time to optimise your visit. Print off floor plans from the Optrafair website and highlight the stands you have decided to visit, then work in a methodical way. Many visitors are wise to exhibitions and try to take in the show over a couple of days, allowing time to attend important lectures and attain valuable CET points along the way. Exhibitions can be fun and should be productive. It's astounding how many new ideas, information sources and innovations will inspire you and contribute to the positive development of your practice and its continued success.
