1 BIOLOGICAL SAFETY INSPECTION CHECKLIST Section A : Contact Information (Principle Investigator) Last Name: First Name: Extension: Department: Building: Room: Section B: Inspection Date of Inspection: Time of Inspection: Section C: Biological Laboratory Work Practices Lab personnel are aware of the location of the Biosafety Manual and Standard Operating Procedures. Evidence to show Lab personnel are required to follow these SOP s. (sign off sheets) No evidence of food or drink being consumed or stored in the lab. It is forbidden to apply cosmetics or insert/remove contact lenses in the laboratory. (written policy) Long hair and loose jewelry are restrained. 6 Open cuts, scratches and/or wounds are covered with appropriate dressings. 7 Syringes and other sharp objects are disposed in labeled sharps container. 8 No evidence of recapping, bending or removal of needle from the syringe. 9 Other AM PM Section D: Laboratory Design, Housekeeping & Physical Requirements Access is limited to authorized personnel. The laboratory is separated from public areas by a door. Aisles and exits are free from obstruction, no tripping or slippery hazard present. Paperwork and report areas are separate from areas where biohazardous materials are manipulated. The door leading to the laboratory is closed and lockable or access controlled. 6 Doors, frames, casework and bench tops are in good condition and non-absorptive. 7 Surfaces are scratch, stain, moisture, chemical and heat resistant. 8 If windows are present, they are equipped with screens or sealed shut. 9 Hand washing sinks and soap are located near laboratory exits. 0 An emergency eyewash station and shower are available inside, or in proximity of, the laboratory. Other
2 Section E: Personal Protective Equipment Personnel (including visitors, trainees and all others) are wearing protective laboratory clothing & foot wear when working in the laboratory. Gloves are worn for all procedures that might involve direct skin contact with biohazardous material or infected animals. Street clothing and laboratory clothing areas are physically separated. There is a mechanism to hang laboratory coats near the exit. Lab coats are only worn in the laboratory and are removed before entering non-laboratory general use area. 6 Respirators are provided when necessary, and selected on the basis of hazard present (users are trained and fit tested) 7 Other Section F: Hazard Communication Current Approved University of Windsor Biohazard permit is posted in the laboratory. Signs on storage areas (level ) and laboratory doors are consistent with the hazards within. Lab personnel know how to get access to MSDS for their work. Biohazardous material is clearly identified and labeled. List of approved personnel are listed and updated on permit. 6 Appropriate warning & hazard signs are posted. 7 Other Section G: Training & Regulatory Records All personnel have received training on the potential hazards associated with the work involved, and on the necessary precautions to prevent potential exposure. (documentation and sign off by employee & lab supervisor) All people working in the containment area have been trained in the biosecurity plan. All personnel listed on the biosafety permit have taken University of Windsor Biosafety Training. (records present) All training is current (Biosafety Level annual online retraining required.) Personnel are trained in all decontamination procedures specific to their activities. 6 Personnel working in a Biosafety Cabinet are trained in its correct use and have a good understanding of the different types of cabinets and uses. 7 Inventory records are updated regularly. 8 Records of regulated materials are kept. 9 Permits for PHAC/CFIA are in use when applicable and reviewed by the BSO for approval. 0 Other
3 Section H: Biological Safety Cabinets Biological Safety Cabinets are certified annually, after repair or relocation. (certification posted) BSCs are located away from high traffic areas, doors and air supplies. UV light is turned off when BSC is in use. Proper BSC procedures are performed when in use. Open flames are avoided when using the BSC. 6 BSC benches are not cluttered and airflow is unrestricted. 7 Work surfaces are decontaminated after use. 8 Waste containers available near BSCs. Other Section I: Decontamination & Waste Handling All contaminated materials are decontaminated before disposal or removal from the laboratory. Effective products are used for the decontamination of equipment, samples, surfaces and spills of infectious materials. Laboratory bench tops and surfaces are decontaminated at the end of the work day or immediately after any spill. Biological waste is appropriately marked and labeled. Sufficient disinfectants effective against infectious materials in use are available at all times within the laboratory. 6 Biohazard autoclave bags are available. 7 Biological Spill Kit is available and all personnel are familiar with location and associated protocols. 7 All solid biological waste is placed in biohazard autoclave bags and autoclaved. 8 Autoclave efficiency is monitored with biological indicators (log records must be available) 9 Liquid biological waste are neutralized prior to disposal 0 Other Section J: Emergencies Emergency procedures for spill cleanup, BSC failure, fire, and other emergencies are written and easily accessible. Records of other people such as emergency responders are kept. All spills, accidents or exposures to infectious materials are reported to the laboratory supervisor and the Biosafety Officer. An Emergency Plan is available to all laboratory personnel. This includes items such as emergency numbers, MSDS information and protocols Other
